拔伸推拿结合牵引和药物治疗椎动脉型颈椎病临床研究

Clinical Observation of Tractive Manipulation Therapy Combined with Traction and medicine on Vertebral Arterial Type of Cervical Spondylosis

目的观察拔伸推拿结合牵引和药物治疗椎动脉型颈椎病的临床疗效。方法以85例上海交通大学医学院附属瑞金医院推拿科2018年12月至2019年12月符合椎动脉型颈椎病诊断患者为观察对象,按随机数字表法分为观察组43例和对照组42例。观察组给予拔伸推拿及牵引和药物治疗,对照组仅给予牵引和药物治疗。共治疗8周,其中观察组脱落3例,对照组脱落2例。治疗前和治疗后观察两组颈源性眩晕症状与功能评分;治疗8周后观察两组临床疗效。结果观察组与对照组治疗前后颈源性眩晕症状与功能评分比较,差异有显著性(P<0.05);观察组与治疗组治疗前后临床疗效评定差异有显著性,且观察组临床疗效优于对照组(P<0.05);结论拔伸推拿结合牵引和药物治疗椎动脉型颈椎病疗效显著,可快速缓解患者颈源性眩晕临床症状,提高生活、工作、心理等功能,最终达到治愈椎动脉型颈椎病的目的。

Objective To observe the therapeutic effect of tractive manipulation therapy combined with traction and medicine in treating patients with vertebral arterial type of cervical spondylosis.Method A total of 85 patients in syndromes of vertebral arterial type of cervical spondylosis in our hospital from Oct 2017 to Jun 2019 were randomized into observation group and controlled group.43 cases in the observation group were treated with tractive manipulation therapy combined with traction and Chinese Medicine,while 42 cases in the controlled group were treated with only traction and Chinese Medicine.3 patients in the observation group and 2 in the controlled group were failed to follow up during the treatment course.The 2 groups were treated for 8 weeks and total clinical effects were evaluated in every end of the week;before and after treatment,scores of evaluations of cervical vertigo symptoms and functions were observed.Result The total effective rate in the observation group was significantly higher than that in the controlled group(P<0.05);and scores of evaluations of cervical vertigo symptoms and functions in the observation group were significantly increased compared with those in the controlled group(P<0.05).Conclusion Tractive manipulation therapy combined with traction and Chinese Medicine had marked effects in treating patients with vertebral arterial type of cervical spondylosis.
