子宫内膜异位症患者IVF-ET结局分析

Analysis of Patients with Endometriosis on Outcomes of IVF-ET

目的:分析子宫内膜异位症对体外受精-胚胎移植(IVF-ET)结局的影响。方法:回顾性选取2016年11月-2020年6月厦门大学附属妇女儿童医院生殖医学科行IVF-ET的247例子宫内膜异位症患者作为子宫内膜异位症组(n=247),同时选取同期691例因输卵管因素(排除子宫内膜异位症因素)不孕而行IVF-ET的患者作为对照组(n=691)。两组均行IVF-ET。比较两组年龄、体质量指数(BMI)、IVF-ET胚胎培养结局、移植胚胎数及妊娠结局。结果:两组年龄比较差异无统计学意义(P>0.05),对照组BMI高于子宫内膜异位症组(P<0.05)。对照组获卵数多于子宫内膜异位症组,子宫内膜异位症组获卵率、成熟卵率(MII卵率)、囊胚利用率均低于对照组(P<0.05)。两组两原核受精率(2PN受精率)、可用胚率、优质胚胎率、优质囊胚率相比差异均无统计学意义(P>0.05)。两组妊娠、种植、活产率、每个胚胎活产率、移植胚胎数比较差异均无统计学意义(P>0.05)。结论:子宫内膜异位症患者卵子质量均有一定程度的下降。

Objective:To analyze the effect of endometriosis on the outcomes of in vitro fertilization and embryo transfer(IVF-ET).Method:247 patients with endometriosis from November 2016 to June 2020 in Department of Reproductive Medicine in Women’s and Children’s Hospital of Xiamen University were selected retrospectively as the endometriosis group(n=247),and 691 infertility patients who underwent IVF-ET due to tubal factors(excluding endometriosis factor)in the same period were selected as the control group(n=691).IVF-ET was performed in both groups.The age,body mass index(BMI),IVF-ET embryo culture outcome,the number of transferred embryos and pregnancy outcome were compared between the two groups.Result:There was no significant difference in age between the two groups(P>0.05).The BMI of the control group was higher than that of the endometriosis group(P<0.05).The number of eggs obtained in the control group was more than that in the endometriosis group,the egg retrieval rate,mature egg rate(MII egg rate)and blastocyst utilization rate in the endometriosis group were lower than those in the control group(P<0.05).There were no significant differences between the two groups in terms of two pronuclear fertilization rate(2PN fertilization rate),available embryo rate,excellent embryo rate and high-quality blastocyst rate(P>0.05).There were no significant differences in pregnancy,implantation,live birth rate and live birth rate of each embryo,the number of transferred embryos between the two groups(P>0.05).Conclusion:Endometriosis patients have a certain degree of egg quality decline.
