桂枝甘草龙骨牡蛎汤加味治疗老年冠心病心律失常的临床效果

Clinical Effect of Guizhi Gancao Longgu Muli Decoction Flavored Treatment in the Arrhythmia of Elderly Patients with Coronary Heart Disease

目的:探究桂枝甘草龙骨牡蛎汤加味治疗老年冠心病心律失常的临床效果。方法:选择惠东县人民医院2016年6月-2020年6月收治的老年冠心病心律失常患者100例为研究对象。采用信封法随机将其分为对照组与治疗组,各50例。对照组给予常规治疗,治疗组在常规治疗基础上给予桂枝甘草龙骨牡蛎汤加味治疗。比较两组临床疗效、治疗前后中医症候积分、心功能[心排出量(CO)、左心室舒张末期内径(LVEDD)与左室射血分数(LVEF)]、血脂[总胆固醇(TC)、三酰甘油(TG)]、超敏C反应蛋白(hs-CRP)、B型利钠肽(BNP)、QT间期离散度(QTd)及不良反应。结果:治疗组总有效率(96.00%)高于对照组(82.00%)(χ^(2)=5.005,P<0.05)。治疗后,治疗组中医症候积分为(8.21±1.96)分,低于对照组的(13.73±3.16)分(t=10.497,P<0.05)。治疗后,治疗组CO、LVEF均高于对照组,LVEDD低于对照组(t=5.305、8.440、7.677,P<0.05)。治疗后,治疗组TC、TG、hs-CRP、BNP水平均低于对照组(t=6.218、7.619、12.706、9.176,P<0.05)。治疗后,治疗组QTd为(51.26±7.32)ms,低于对照组的(60.37±8.48)ms(t=5.750,P<0.05)。两组治疗期间均未发生严重不良反应。结论:桂枝甘草龙骨牡蛎汤加味治疗老年冠心病心律失常效果显著,可改善患者心功能,降低血脂,调控hs-CRP、BNP水平,降低QTd。

Objective:To explore the clinical effect of Guizhi Gancao Longgu Muli Decoction flavored treatment in arrhythmia of elderly patients with coronary heart disease.Method:A total of 100 arrhythmia of elderly patients with coronary heart disease admitted to Huidong County People’s Hospital from June 2016 to June 2020 were selected as the research objects.They were randomly divided into control group and treatment group by envelope method,with 50 cases in each group.The control group was given conventional treatment,and the treatment group was given Guizhi Gancao Longgu Muli Decoction flavored treatment on the basis of conventional treatment.The clinical efficacy,TCM syndrome score,cardiac function[cardiac output(CO),left ventricular end-diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF)],blood lipid[total cholesterol(TC),triglyceride(TG)],hypersensitive C-reactive protein(hs-CRP),B-type natriuretic peptide(BNP),QT interval dispersion(QTd)before and after treatment and adverse reactions of the two groups were compared.Result:The total effective rate in treatment group(96.00%)was higher than that in control group(82.00%)(χ^(2)=5.005,P<0.05).After treatment,the TCM syndrome score of the treatment group was(8.21±1.96)points,which was lower than(13.73±3.16)points of the control group(t=10.497,P<0.05).After treatment,CO and LVEF in treatment group were higher than those in control group,LVEDD was lower than that in control group(t=5.305,8.440,7.677,P<0.05).After treatment,TC,TG,hs-CRP and BNP levels in the treatment group were lower than those in the control group(t=6.218,7.619,12.706,9.176,P<0.05).After treatment,QTd of the treatment group was(51.26±7.32)ms,which was lower than(60.37±8.48)ms of the control group(t=5.750,P<0.05).No serious adverse reactions occurred in the two groups during treatment.Conclusion:The Guizhi Gancao Longgu Muli Decoction flavored treatment has a significant effect in arrhythmia of elderly patients with coronary heart disease,which can improve the cardiac function of patients,reduce blood lipid,regulate hs-CRP,BNP levels,and reduce QTd.
