单孔胸腔镜肺癌根治术后患者疼痛相关影响因素分析

Analysis of Related Factors of Pain in Patients with Lung Cancer after Single-port Thoracoscopic Radical Operation

目的:分析单孔胸腔镜肺癌根治术后患者疼痛的相关影响因素。方法:回顾性选取70例2016年1月-2021年3月于潜江市中心医院胸外科行单孔胸腔镜肺癌根治术治疗的肺癌患者为研究对象,术后以疼痛数字评分法(NRS)评估患者术后疼痛程度,据其可分为疼痛达标组(NRS<3分,40例)、疼痛未达标组(NRS≥3分,30例)。收集并比较两组的临床资料,同时以多因素Logistic回归分析法分析单孔胸腔镜肺癌根治术后患者疼痛的影响因素。结果:疼痛未达标组年龄≥70岁、切口肋间隙宽度<1.5 cm、术毕镇痛方法为静脉镇痛泵患者占比高于疼痛达标组(P<0.05)。多因素Logistic分析结果显示,年龄≥70岁、切口肋间隙宽度<1.5 cm、术毕镇痛方法为静脉镇痛泵为影响单孔胸腔镜肺癌根治术后患者疼痛的危险因素(OR=2.022、2.022、1.815,P<0.05)。结论:年龄大小、切口肋间隙宽度、术毕镇痛方法均是影响单孔胸腔镜肺癌根治术后患者疼痛的高危因素。

Objective:To analyze the influencing factors of pain in patients with single-port thoracoscopic radical operation.Method:A total of 70 cases of lung cancer treated with single-hole thoracoscopic lung cancer in thoracic surgery of Qianjiang Central Hospital from January 2016 to March 2021 were retrospectively selected.After surgery,the pain was assessed by the numerical rating scale(NRS)method.They were divided into the pain standard group(NRS<3 points,40 cases)and the pain failure group(NRS≥3 points,30 cases)according to NRS.The clinical data of the two groups were collected and compared.At the same time,multi-factor Logistic regression analysis was used to analyze the influencing factors of pain in patients with single-port thoracoscopic radical operation after radical resection.Result:The proportion of age≥70 years,<1.5 cm of incision intercostal space width,the analgesic method after operation with intravenous analgesia pump in the pain failure group were higher than those of pain standard group(P<0.05).A multivariate Logistic analysis showed,the age≥70 years,the width of incision intercostal space<1.5 cm,the analgesic method after operation with intravenous analgesia pump were the risk factor for postoperative pain in patients with single-port thoracoscopic radical operation(OR=2.022,2.022,1.815,P<0.05).Conclusion:Age,width of incision intercostal space,postoperative analgesia methods are all high-risk factors that affect the pain of patients after radical resection of lung cancer by single-hole thoracoscopy.
