摘要
目的以项目管理形式构建和实施疾病应急救助管理系统,降低急救绿色通道造成的费用损失,保障需要紧急救治的特殊群体患者生命安全及健康。方法参照《国务院办公厅关于建立疾病应急救助制度的指导意见》、《关于做好疾病应急救助有关工作的通知》等文件的要求,回顾该管理项目建设的全过程,通过根因和风险因素分析,分解责任范围,充分考虑各相关者的利益,加深多个部门间的配合和沟通,临床积极掌握需要紧急救治的急、危、重伤病标准及诊疗规范,积极救治"三无"患者。结果总结经验教训,通过表格工具,发现经验管理中隐藏的问题提前规避或解决,各部门了解、掌握政策,规范实施首诊负责等核心制度,最大程度缓解急救欠费压力。结论完成疾病应急救助工作的同时,通过项目管理提供了正确的思维方式、必要的技能和恰当的工具,有效改善患者的就医体验,切实保障广大人民群众的身体健康权益,体现医院的公益性。
Objective To construct and implement the disease emergency relief management system in the form of project management,reduce the cost loss caused by the green channel of first aid,and ensure the life safety and health of special groups of patients in need of emergency treatment. Methods With reference to the requirements from documents,such as the Guidance from the General Office of the State Council on Establishing a Disease Emergence Rescue System,the Notice of Conducting Good Disease Emergence Rescue,we reviewed the whole process of management of project construction,and through root cause analysis and risk factor analysis,disassembled scope of responsibility,gave full consideration to the interests of all stakeholders,deepened cooperation and communication between multiple departments. Clinically,we actively mastered the standards and diagnosis and treatment specification of acute,critical and serious diseases requiring emergency treatment,and actively treated the "three-non patients". Results We summarized the experience and lessons,found the hidden problems in experience management to avoid or solve in advance through the form tool. All departments understood and grasped the policy and standardized the implementation of the core system such as responsibility for the first diagnosis,so as to relieve the pressure of arrears first-aid fees to the greatest extent. Conclusion While completing the work of disease emergency relief,we provided the correct way of thinking,necessary skills and appropriate tools through project management to effectively improve the patient’s medical experience,effectively protect the health rights and interests of the masses of people,and reflect the public welfare of the hospital.
