系统化营养管理在慢性肾脏病患者中的应用效果分析 被引量:4

Effectiveness of nutrition management based on information system in patients with chronic kidney disease

摘要 目的探讨营养管理在慢性肾脏病患者中的应用效果。方法选取2015年1月至2019年12月在江苏省中医院和金堂县第一人民医院就诊的534名患者,将加入营养管理的患者纳入为干预组(259例),未加入营养管理的患者纳入为对照组(275例),进行6个月和12个月的随访,比较两组患者的血红蛋白、血清白蛋白、肾小球滤过率、血肌酐和尿酸水平。结果随访6个月及12个月时,干预组血红蛋白和血清白蛋白水平均显著高于对照组(P<0.05),且随访6个月时,干预组血红蛋白水平也显著高于对照组(P<0.05);随访12个月时两组血清白蛋白水平差异无统计学意义(P>0.05);随访6个月及12个月时,干预组肾小球滤过率显著高于对照组(P>0.05),而干预组血肌酐和尿酸水平则显著低于对照组(P>0.05)。结论对慢性肾脏病患者进行营养管理能够显著改善患者的营养状况,延缓肾脏疾病的发展。 Objective To explore the effect of nutritional management in patients with chronic kidney disease.Methods A total of 534 patients admitted to Jiangsu Provincial Hospital of Traditional Chinese Medicine and the First People’s Hospital of Jintang from January 2015 to December 2019 were selected.The 259 patients enrolled in nutritional management were included in the observation group,and 275 patients who did not enroll in nutritional management were included in the control group.The patients were followed up for 6 months and 12 months.The levels of hemoglobin,albumin,glomerular filtration rate,serum creatinine and uric acid of the two groups were compared.Results After 6 months and 12 months follow-up,the levels of hemoglobin and serum albumin in the observation group were significantly higher than those in the control group(P<0.05),and the hemoglobin level in the observation group was significantly higher than that in the control group at 6 months follow-up(P<0.05),while there was no significant difference in serum albumin level between the two groups after 12 months follow-up(P>0.05).At 6 and 12 months of follow-up,GFR in the observation group was significantly higher than that in the control group(P<0.05),while serum creatinine and uric acid in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Nutritional management for patients with chronic kidney disease can significantly improve the patients’ nutritional status and delay the development of kidney disease.
