光固化复合树脂修复牙体楔状缺损临床效果的影响因素分析

Influencing factors of repair effect of light curing composite resin on wedge-shaped defect of teeth

目的探讨光固化复合树脂修复牙体楔状缺损临床效果的影响因素。方法抽取2018年2月至2020年2月在驻马店市第二中医院接受光固化复合树脂修复治疗的牙体楔状缺损患者123例(患牙614颗)作为研究对象,全部患者均于治疗结束后随访1年,依据随访结果分为失败组和成功组。设计基线资料调查表,分析患者基线资料情况,并经单因素及多因素分析光固化复合树脂修复牙体楔状缺损患者临床效果的影响因素。结果123例楔状缺损患者中修复失败19例,纳入失败组;其余104例纳入成功组。失败组复合树脂、偏侧咀嚼、不良刷牙习惯占比均高于成功组,牙龈沟液pH值低于成功组,差异有统计学意义(P<0.05);两组其他资料比较差异未见统计学意义(P>0.05)。多元Logistic回归分析结果显示,复合树脂、偏侧咀嚼、不良刷牙习惯为牙体楔状缺损患者光固化复合树脂治疗效果的危险因素(OR>1,P<0.05),牙龈沟液pH值高为楔状缺损患者光固化复合树脂治疗效果的保护因素(OR<1,P<0.05)。结论光固化复合树脂修复牙体楔状缺损有一定的效果,修复失败的危险因素为复合树脂填充、偏侧咀嚼、不良刷牙习惯,修复效果的保护因素为牙龈沟液pH值高。

Objective To investigate the influencing factors of repair effect of light curing composite resin in the treatment of wedge-shaped defect of teeth.Methods A total of 123 patients(614 teeth)with wedge-shaped tooth defects treated with light cured composite resin in the Zhumadian Second Hospital of Traditional Chinese Medicine from February 2018 to February 2020 were selected as the research objects.All patients were followed up for 1 year after treatment,the patients were divided into failure group and success group according to the repair effects.The baseline data questionnaire was designed to analyze the baseline data of patients.Univariate and multivariate analysis were used to analyze the influencing factors of the therapeutic effect of light cured composite resin in patients with wedge-shaped defects.Results In 123 patients with wedge-shaped defects,19 cases failed to repair,and they were included into the failure group;the other 104 cases were included into the success group.The proportion of composite resin,unilateral mastication and bad brushing habits in failure group were higher than those in success group,and the pH value of gingival crevicular fluid in failure group was lower than that in success group(P<0.05);there was no statistical significant difference in other data between the two groups(P>0.05).The results of multivariate Logistic regression analysis showed that composite resin,unilateral chewing and bad brushing habits were the risk factors for the therapeutic effect of light cured composite resin in patients with wedge-shaped defects(OR>1,P<0.05),while high pH value of gingival crevicular fluid was a protective factor for the therapeutic effect of light cured composite resin in patients with wedge-shaped defect(OR<1,P<0.05).Conclusions Light curing composite resin has certain effects in the treatment of wedge-shaped defects.Composite resin filling,unilateral chewing,bad brushing habits were possible risk factors,and the high pH value of gingival crevicular fluid was the protective factor.
