骨髓增殖性肿瘤中JAK2V617F突变与Ⅰ型细胞因子受体的相关性 被引量:2

摘要 目的探讨骨髓增殖性肿瘤(MPN)中Ⅰ型细胞因子受体与JAK2V617F突变之间的相关性。方法选择2018年1月-2021年4月沧州市中心医院收治的MPN患者50例设为MPN组,另选择同期健康者45例设为正常对照组和15例慢性粒细胞白血病(CML)患者设为CML组,运用定量逆转聚合酶链反应(FQ-RT-PCR)技术对受检者外周血或骨髓JAK2V617F突变及Ⅰ型细胞因子受体[血小板生成素受体(c-Mpl)、粒细胞集落刺激因子受体(GCSFR)以及促红细胞生成素受体(EPOR)]的mRNA表达情况进行检测。结果MPN组中30例存在JAK2V617F点突变,占比60.00%,其中PMF患者、ET患者以及PV患者的阳性率分别为53.33%、73.33%、55.00%,但正常对照组和CML组无JAK2V617F突变情况。CML组c-Mpl、GCSFR、EPOR的mRNA表达水平均高于MPN组、正常对照组,差异有统计学意义(P<0.05);MPN组GCSFR、EPOR的mRNA表达水平均高于正常对照组,差异有统计学意义(P<0.05);但MPN组与正常对照组c-Mpl的mRNA表达水平比较,差异无统计学意义(P>0.05);MPN组中JAK2V617F突变阴性初治和JAK2V617F突变阳性初治患者的c-Mpl、GCSFR以及EPOR的mRNA表达比较,差异无统计学意义(P>0.05)。结论JAK2V617F和Ⅰ型细胞因子参与MPN的发展,并且在多种造血系统肿瘤疾病中Ⅰ型细胞因子的表达水平较高,但并不是决定MPN发病的唯一因素。 Objective To investigate the correlation between typeⅠcytokine receptor and JAK2V617F mutation in myeloproliferative neoplasms(MPN).Methods A total of 50 patients with MPN admitted to Cangzhou Central Hospital from January 2018 to April 2021 were selected as MPN group,and 45 healthy people were selected as normal control group and 15 patients with chronic myeloid leukemia(CML)were selected as CML group.The quantitative reverse polymerase chain reaction(FQ-RT-PCR)technique was used to detect the JAK2V617F mutation and the mRNA expression of typeⅠcytokines[thrombopoietin receptor(c-Mpl),granulocyte colony stimulating factor receptor(GCSFR)and erythropoietin receptor(EPOR)]in peripheral blood or bone marrow of the subjects.Results There were 30 patients with JAK2V617F mutation in MPN group,accounting for 60.00%.The positive rates of PMF,ET and PV patients were 53.33%,73.33%and 55.00%,respectively;there was no JAK2V617F mutation in normal control group and CML group.The mRNA expression levels of c-Mpl,GCSFR and EPOR in CML group were higher than those in MPN group and normal control group,and the difference was statistically significant(P<0.05);the mRNA expression levels of GCSFR and EPOR in MPN group were higher than those in the normal control group,and the difference was statistically significant(P<0.05);however,there was no significant difference in c-Mpl mRNA expression between MPN group and normal control group(P>0.05).There was no significant difference in the mRNA expression of c-Mpl,GCSFR and EPOR between the patients with JAK2V617F mutation negative untreated and JAK2V617F mutation positive untreated in MPN group(P>0.05).Conclusion JAK2V617F and typeⅠcytokines are involved in the development of MPN,and the expression level of typeⅠcytokines is high in a variety of hematopoietic tumor diseases,but it is not the only factor that determines the pathogenesis of MPN.
