高敏肌钙蛋白T在2型糖尿病患者中的分布特点及预后检测价值分析

Distribution Characteristics and Prognostic Value of High-sensitivity Troponin T in Patients with Type 2 Diabetes Mellitus

目的分析高敏肌钙蛋白T(hs-cTnT)在2型糖尿病(T2DM)患者中的分布特点及预后检测价值。方法选取2019年4月-2021年4月我院确诊的186例T2DM患者为观察对象,另选取200例非T2DM体检者进行对照,分析hs-cTnT的分布特点及检测价值。结果T2DM患者的hs-cTnT检出率及升高率均高于非T2DM者,差异有统计学意义(P<0.05);不同hs-cTnT水平的T2DM患者年龄、性别、吸烟、冠心病史、高血压史、血压水平、血糖水平、eGFR及hs-CRP比较(P<0.05);hs-cTnT与年龄、血压水、血糖及hs-CRP水平呈正相关,与eGFR呈负相关(P<0.05);多因素逐步回归分析显示,年龄、吸烟、血糖水平、eGFR、hs-CRP指标均是引起hs-cTnT升高的危险因素,且hs-cTnT>14 ng/L的T2DM患者心血管事件发生率高于hs-cTnT 3~14 ng/L及hs-cTnT<3 ng/L患者(P<0.05)。结论hs-cTnT与T2DM患者的心血管事件危险因素显著相关,风险预测价值高。

Objective To analyze the distribution characteristics and prognostic value of high-sensitivity troponin T(hs-cTnT)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 186 patients with T2DM diagnosed from April 2019 to April 2021 of our hospital were selected as observation objects,and 200 non-T2 DM subjects were selected as control.The distribution characteristics and detection value of hs-cTnT were analyzed.Results The detection rate and elevation rate of hs-cTnT in T2DM patients were higher than those in non-T2DM patient(P<0.05).There were statistically significant differences in age,gender,smoking,history of coronary heart disease,history of hypertension,blood pressure level,blood glucose level,eGFR and hs-CRP among T2DM patients with different hs-cTnT levels(P<0.05).hs-cTnT was positively correlated with age,blood pressure,blood glucose and hs-CRP levels,and negatively correlated with eGFR(P<0.05).Multivariate stepwise regression analysis showed that age,smoking,blood glucose level,eGFR and hs-CRP were all risk factors for the increase of hs-cTnT,and the incidence of cardiovascular events in T2DM patients with hs-cTnT>14 ng/L was higher than that in patients with hs-cTnT 3-14 ng/L and hs-cTnT<3 ng/L(P<0.05).Conclusion hs-cTnT is significantly correlated with the risk factors of cardiovascular events in patients with T2DM,and the risk prediction value is high.
