标准大骨瓣开颅减压术治疗颅脑外伤的临床效果

Clinical effect of standard large bone flap craniotomy with craniotomy for treatment of craniocerebral trauma

目的探究标准大骨瓣开颅减压术治疗颅脑外伤患者的临床效果。方法选取2018年1月至2021年1月本院收治的70例颅脑外伤患者作为研究对象,随机分为观察组与对照组,每组35例。对照组采取常规去骨瓣减压术治疗,观察组采取标准大骨瓣开颅减压术治疗,比较两组术后颅内压水平、神经功能指标及临床疗效。结果术后1、3、7 d,观察组术后颅内压水平均低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组NGF均高于治疗前,GFAP、S-100β均低于治疗前,且观察组NGF高于对照组,GFAP、S-100β均低于对照组,差异有统计学意义(P<0.05)。结论标准大骨瓣开颅减压术治疗颅脑外伤效果显著,能降低术后颅内压水平,改善患者神经功能,值得临床推广应用。

Objective To explore the clinical effect of standard large bone flap craniotomy in the treatment of patients with craniocerebral trau-ma.Methods 70 patients with craniocerebral trauma admitted to our hospital from January 2018 to January 2021 were selected as the research sub-jects,and they were randomly divided into observation group and control group,with 35 cases in each group.The control group was treated with con-ventional decompression craniectomy,and the observation group was treated with standard large bone flap craniotomy.The postoperative intracranial pressure level,neurological function indexes and clinical efficacy were compared between the two groups.Results After 1,3,and 7 d postoperative-ly,the postoperative intracranial pressure in the observation group was lower than that in the control group,and the difference was statistically signifi-cant(P<0.05).The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statisti-cally significant(P<0.05).After treatment,NGF of the two groups were higher than before treatment,GFAP and S-100βwere lower than before treatment,and NGF of the observation group was higher than that of the control group,and GFAP and S-100βwere lower than those of the control group,the difference was statistically significant(P<0.05).Conclusion The standard large bone flap craniotomy has significant effect on the treat-ment of craniocerebral trauma,it can reduce the level of intracranial pressure after surgery and can effectively improve the neurological function of patients,it is worthy of clinical promotion and application.
