不同麻醉方式在小儿纤维支气管镜检查中的效果比较

Comparison of the effect of different anesthesia methods in pediatric fiberoptic bronchoscopy

目的探究不同麻醉方式在小儿纤维支气管镜检查中的效果比较。方法选取2018年10月至2020年10月于本院行纤维支气管镜检查的80例患儿,按照随机数字表法分为对照组与实验组,每组40例。对照组行丙泊酚联合瑞芬太尼静脉麻醉,实验组行七氟醚吸入麻醉。比较两组免疫功能、不良反应发生情况及血清γ干扰素(IFN-γ)、白介素-4(IL-4)、IFN-γ/IL-4水平。结果实验组不良反应发生率为5.00%,低于对照组的22.50%,差异有统计学意义(P<0.05)。实验组外周血CD4^(+)、CD8^(+)、血清IFN-γ、IL-4水平及CD4^(+)/CD8^(+)比值均低于对照组,IFN-γ/IL-4比值高于对照组,差异有统计学意义(P<0.05)。结论与丙泊酚联合瑞芬太尼静脉麻醉相比,在小儿纤维支气管镜检查中,七氟醚吸入麻醉可调节免疫功能,降低炎症反应,具有较高的安全性。

Objective To explore the effect comparison of different anesthesia methods in pediatric fiberoptic bronchoscopy.Methods 80 chil-dren who underwent fiberoptic bronchoscopy in our hospital from October 2018 to October 2020 were selected and they were divided into control group and experimental group according to the random number table method,with 40 cases in each group.The control group received propofol com-bined with remifentanil intravenous anesthesia,and the experimental group received sevoflurane inhalation anesthesia.The immune function,adverse reactions and serum levels of interferon gamma(IFN-γ),interleukin-4(IL-4)and IFN-γ/IL-4 were compared between the two groups.Results The in-cidence of adverse reactions in the experimental group was 5.00%,which was lower than 22.50%in the control group,and the difference was statis-tically significant(P<0.05).The peripheral blood CD4^(+),CD8^(+),serum IFN-γ,IL-4 levels and CD4^(+)/CD8^(+)ratio in the experimental group were low-er than those in the control group,and the IFN-γ/IL-4 ratio was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with propofol combined with remifentanil intravenous anesthesia,sevoflurane inhalation anesthesia can regulate immune function and reduce inflammatory response in pediatric fiberoptic bronchoscopy with higher safety.
