高血压脑出血偏瘫患者病耻感及社会支持度影响因素分析

Analysis of influencing factors of stigma and social support in hemiplegic patients with hypertensive cerebral hemorrhage

目的探讨高血压脑出血偏瘫患者病耻感及社会支持度影响因素分析。方法对92例高血压脑出血偏瘫患者采用自拟一般资料调查表统计其一般资料,采用脑卒中患者病耻感测评量表及社会支持评定量表评定患者的病耻感及社会支持度。采用单因素及多元线性回归分析探讨高血压脑出血偏瘫患者社会支持度的影响因素。结果本组高血压脑出血偏瘫患者社会支持评定量表评分为(34.51±3.42)分,脑卒中患者病耻感测评量表评分为(63.72±5.09)分。不同年龄、文化程度、婚姻状况、居住地、家庭月收入、偏瘫程度、主要照顾者、病耻感水平的高血压脑出血偏瘫患者社会支持评定量表评分比较差异有统计学意义(P<0.01)。Pearson相关分析显示,高血压脑出血偏瘫患者社会支持评定量表评分与脑卒中患者病耻感测评量表评分呈显著负相关(r=-0.359,P<0.01)。多元线性回归分析显示,年龄、文化程度、婚姻状况、居住地、家庭月收入、偏瘫程度、主要照顾者、病耻感水平均为高血压脑出血偏瘫患者社会支持水平的影响因素(P<0.01)。结论高血压脑出血偏瘫患者病耻感较强,社会支持度一般但影响因素众多,且与病耻感密切相关,医护人员应重视并对其予以有针对性的干预。

Objective To investigate the influencing factors of stigma and social support in hemiplegic patients with hypertensive cerebral hemorrhage.Methods The general data of 92 cases of hemiplegic patients with hypertensive cerebral hemorrhage were collected by self-made general data questionnaire.The stigma and social support of patients were assessed by the stigma scale for stroke(SSS)and social support rating scale(SSRS).Univariate and multiple linear regression analysis were used to explore the influencing factors of social support in hemiplegic patients with hypertensive cerebral hemorrhage.Results The SSRS and SSS scores of hemiplegia patients with hypertensive cerebral hemorrhage were(34.51±3.42)and(63.72±5.09)points.There were significant differences in the SSRS scores of hemiplegic patients with hypertensive cerebral hemorrhage with different age,education level,marital status,place of residence,monthly family income,degree of hemiplegia,primary caregiver,and stigma level(P<0.01).Pearson correlation analysis showed that there was a significant negative correlation between SSRS score and SSS score in hemiplegic patients with hypertensive cerebral hemorrhage(r=-0.359,P<0.01).Multiple linear regression analysis showed that age,educational level,marital status,place of residence,monthly family income,degree of hemiplegia,main caregiver and stigma level were the influencing factors of social support level of hemiplegic patients with hypertensive intracerebral hemorrhage(P<0.01).Conclusions Hemiplegic patients with hypertensive intracerebral hemorrhage have a strong sense of stigma and general social support,but there are many influencing factors,which are closely related to the sense of stigma.Medical staff should pay attention to it and give targeted interventions.
