动态心电图诊断心肌缺血的临床价值分析

Clinical Diagnostic Value of Ambulatory Electrocardiogram in the Diagnosis of Myocardial Ischemia

目的:探讨动态心电图诊断心肌缺血的临床价值。方法:选取2019年1月-2020年12月收治的冠心病心肌缺血患者168例,随机数字表法分为两组,每组84例。对照组常规心电图检查,研究组动态心电图检查。比较两组阴性诊断率、阳性诊断率、心律失常诊断情况以及心率变异性频率。结果:研究组阴性诊断率低于对照组,但差异无统计学意义(P>0.05)。研究组阳性诊断率以及房室传导阻滞、短阵室上速、室性心律失常、房性心律失常诊断率均高于对照组,差异有统计学意义(P<0.05);研究组心率变异性频率各指标均大于对照组,差异有统计学意义(P<0.05)。结论:相较于常规心电图诊断,动态心电图诊断冠心病心肌缺血效果更佳,阳性诊断率更高,是一种具有较高应用价值的诊断方式,可为患者后续治疗方案的制定提供科学有效的数据基础。

Objective:To investigate the clinical value of ambulatory electrocardiogram in the diagnosis of myocardial ischemia.Methods:A total of 168 patients with coronary heart disease and myocardial ischemia from January 2019 to December 2020 were selected and divided into two groups according to random number table method,with 84 cases in each group.The control group underwent routine electrocardiogram examination,and the study group underwent ambulatory electrocardiogram examination.The negative diagnosis rate and positive diagnosis rate,the diagnosis of arrhythmia and the frequency of heart rate variability were compared between the two groups.Results:The negative diagnosis rate of the study group was lower than that of the control group,the difference was not significant(P>0.05).The positive diagnosis rate and diagnostic rates of atrioventricular block,short superior ventricular tachycardia,ventricular arrhythmia and atrial arrhythmia in the study group were higher than those in the control group,the difference was significant(P<0.05).The heart rate variability frequency indexes in the study group were higher than those in the control group,the difference was significant(P<0.05).Conclusion:Compared with conventional electrocardiogram,ambulatory electrocardiogram has better diagnostic effect and higher positive diagnosis rate in the diagnosis of myocardial ischemia of coronary heart disease.It is a diagnostic method with high application value and can provide scientific and effective data basis for the formulation of subsequent treatment plan for patients.
