细胞外信号调节激酶激酶5在膀胱尿路上皮癌中的表达及与其预后的相关性

Analysis on expression of MEK5 in bladder urothelial carcinoma and its prognostic correlation

摘要 目的检测细胞外信号调节激酶激酶5(MEK5)在膀胱尿路上皮癌(BUC)中的表达情况,并探讨与其预后的相关性。方法选取2016年1月至2017年12月本院收治的113例膀胱癌患者的临床资料,术中收集癌灶组织113例,癌旁组织70例,记录患者性别、年龄、肿瘤数目、肿瘤大小、病理分级、临床分期、淋巴结转移等信息。采用免疫组织化学法检测组织中MEK5表达情况,比较癌灶组织与癌旁组织MEK5表达的差异,并分析其与BUC患者临床病理特征的关系;采用乘积极限法(Kaplan-Meier)分析BUC患者24个月的预后状况,并采用Log-Rank检验进行显著性比较;采用Cox模型分析BUC患者不良预后的影响因素。结果与癌旁组织相比,癌灶组织的MEK5阳性表达率较高(P<0.05);BUC组织中MEK5表达与性别、年龄、肿瘤数目、肿瘤大小均无相关性(均P>0.05),与病理分级、临床分期、淋巴结转移均有相关性(均P<0.05);与MEK5阴性表达相比,MEK5阳性表达的终点事件发生率较高(39.13%vs.68.66%,P=0.002),无进展生存期较低(21.512个月vs.19.294个月,P=0.001);MEK5阳性表达、肿瘤直径≥3 cm、高级别病理分级、T2~T4期、伴淋巴结转移均是影响BUC患者预后的独立危险因素(P<0.05)。结论BUC患者癌灶组织中MEK5阳性表达率较高,与患者临床病理特征密切相关,可为评估患者病情及预后提供临床参考依据。 Objective To detect the expression of mitogen/extracellular signal-regulated kinase kinase-5(MEK5)in bladder urothelial carcinoma(BUC)and to explore its prognostic correlation.Methods From January 2016 to December 2017,113 patients with bladder cancer were selected for the study.During the operation,113 cancer focus tissues and 70 paracancerous tissues were collected.The information of patients'sex,age,tumor number,tumor size,pathological grade,clinical stage and lymph node metastasis were recorded.The expression of MEK5 was detected by immunohistochemistry,the difference of MEK5 expression between cancerous tissues and adjacent tissues was compared,and its relationship with clinicopathological characteristics of patients with BUC were analyzed;the 24-month prognosis of patients with BUC was analyzed by product limit method(Kaplan-Meier),Log-Rank test was used to compare the significance;and Cox model was used to analyze the influencing factors of poor prognosis in patients with BUC.Results The positive expression rate of MEK5 in cancerous tissues of BUC patients was higher than that in adjacent tissues(P<0.05);the expression of MEK5 in BUC was not related to gender,age,tumor number and tumor size(P>0.05),but to pathological grade,clinical stage and lymph node metastasis(P<0.05);compared with MEK5 negative patients,the incidence of end point events in MEK5 positive patients was higher(P<0.05),and the progression free survival was lower(P<0.05);MEK5 positive expression,tumor≥3 cm,high grade,T2-T4 stage and lymph node metastasis were independent risk factors for prognosis of patients with BUC(P<0.05).Conclusions The positive expression rate of MEK5 in BUC tissue of patients with BUC is high,which is closely related to the clinicopathological characteristics of patients.It is an independent risk factor for poor prognosis of patients,and may provide clinical reference for the evaluation of patients'condition and prognosis.
