女性糖尿病患者泌尿系统感染的尿液菌群特征、血清CRP水平及危险因素分析 被引量:5

Analysis of the characteristics of urinary tract bacteria,serum CRP level and risk factors in female diabetic patients with urinary tract infection

摘要 目的分析女性糖尿病患者泌尿系统感染的尿液菌群特征、血清C反应蛋白(CRP)水平,并探讨患者泌尿系统感染发生的危险因素。方法选择2019年6月至2020年10月本院收治的79例女性糖尿病患者作为研究对象,并依照患者是否并发泌尿系统感染分为感染组(19例)和未感染组(60例);详细收集患者的临床资料,并检测患者的尿液菌群分布特征,采集患者空腹静脉血并检测患者血中CRP水平;采用logistic回归分析影响患者泌尿系统感染发生的危险因素。结果感染组患者的年龄、糖尿病病程、空腹血糖、餐后血糖、糖化血红蛋白、白蛋白、泌尿系结石发生率均明显高于未感染组,血肌酐、血尿素氮明显低于未感染组,且差异均有统计学意义(均P<0.05);感染组患者的OUT、Sobs指数、Chao指数和Ace指数、CRP水平明显高于未感染组,且差异均有统计学意义(均P<0.05),感染组和未感染组患者的Shannon系数及Simpson指数水平比较,差异均无统计学意义(均P>0.05);年龄、糖尿病病程、泌尿系结石、卧床时间、CRP水平、尿液菌群丰度是影响女性糖尿病患者泌尿系统感染的独立性因素。结论女性糖尿病合并泌尿系统感染患者的尿液菌群丰富度、CRP水平显著升高,且年龄、糖尿病病程、泌尿系结石、CRP水平是影响女性糖尿病患者泌尿系统感染的独立性因素。 Objective To analyze the characteristics of urinary tract bacteria and serum C-reaction protein(CRP)level of urinary system infection in female diabetic patients,and to explore the risk factors of urinary system infection.Methods From June 2019 to October 2020,79 female diabetic patients who were admitted to our hospital were selected as the research object,and divided into infection group(19 cases)and uninfected group(60 cases)according to whether the patients were complicated with urinary system infection;the clinical data of patients were collected in detail,and the distribution characteristics of urine bacteria were detected,and the fasting vena blood was collected and CRP level was detected;the influence was analyzed by logistic regression model risk factors of urinary tract infection in patients.Results Age,course of diabetes,fasting blood glucose,postprandial blood glucose,glycosylated hemoglobin,albumin and urinary calculi incidence in the infected group were significantly higher than those in the uninfected group,while serum creatinine and blood urea nitrogen were significantly lower than those in the uninfected group,with statistical significance(all P<0.05).The OUT index,Sobs index,Chao index,Ace index and CRP levels in the infected group were significantly higher than those in the uninfected group,and the differences were statistically significant(all P<0.05).There were no statistically significant differences in Shannon's coefficient and Simpson index levels between the infected group and the uninfected group(all P>0.05).Age,duration of diabetes,urinary calculi,bed time,CRP level and abundance of urine microflora were independent factors affecting urinary tract infection in female diabetic patients.Conclusions The level of urine bacteria abundance and CRP in the infected group increase significantly,and the age,course of diabetes,urinary calculi and CRP level are the independent factors influencing urinary tract infection in women with diabetes mellitus.
