子痫前期孕妇血清补体C1亚单位、C8 β链及H因子相关蛋白5变化及意义

Changes and Significance of Serum Complement C1 Subunit,C8βChain and Complement Factor H Related Protein 5 in Pregnant Women with Preeclampsia

摘要 目的探讨子痫前期孕妇血清补体C1亚单位(C1q)、C8β链(C8β)及H因子相关蛋白5(CFHRP5)变化及意义。方法选取2020年2月至2021年2月在本院首次进行产检的120例孕妇。收集孕妇的一般相关资料,在20~24周对患者进行血压监测,根据妊娠期高血压诊断标准将孕妇分为子痫前期组(n=48)和单纯高血压组(n=72),根据子痫前期组病情程度分为非重度组(n=32)和重度组(n=16),检测入试者血清补体C1q、C8β、CFHRP5、超敏C反应蛋白(hs-CRP)、血管内皮生长因子(VEGF),分析各指标在孕妇机体变化的意义。结果子痫前期组中年龄≥35岁、有家族遗传史以及BMI≥24 kg/m2的人数占比均大于单纯高血压组,收缩压和舒张压均高于单纯高血压组(P<0.05);子痫前期组血清C1q、C8β、VEGF水平均低于单纯高血压组,CFHRP5、hs-CRP水平均高于单纯高血压组(P<0.05);非重度组血清C1q、C8β、VEGF表达水平均高于重度组,CFHRP5、hs-CRP水平均低于重度组(P<0.05);ROC曲线分析结果显示,C1q、C8β、CFHRP5、hs-CRP、VEGF及其联合检测均有预测子痫前期的价值(P<0.05);血清C1q、C8β、VEGF水平与子痫前期严重程度均呈负相关(P值均<0.05),CFHRP5、hs-CRP水平与子痫前期严重程度均呈正相关(P值均<0.05)。结论血清C1q、C8β、CFHRP5与hs-CRP、VEGF联合检测均具有预测子痫前期的价值,且与子痫前期的发展具有相关性。 Objective To explore the changes and significance of serum complement C1 subunit(C1q),C8βchain(C8β)and complement factor H related protein 5(CFHRP5)in pregnant women with preeclampsia.Methods A total of 120 pregnant women undergoing the first antenatal examination in the hospital were enrolled between February 2020 and February 2021.Their general data were collected.The monitoring of blood pressure was conducted within 20-24 weeks.According to the diagnostic criteria of gestational hypertension,they were divided into the preeclampsia group(n=48)and the simple hypertension group(n=72).According to disease severity,patients in the preeclampsia group were divided into the non-severe group(n=32)and the severe group(n=16).The levels of serum complement C1q,C8β,CFHRP5,high-sensitivity C-reactive protein(hs-CRP)and vascular endothelial growth factor(VEGF)were detected,the significance of their changes were analyzed.Results The proportions of cases with age not younger than 35 years,family inheritance history and BMI not lower than 24k g/m2 in the preeclampsia group were higher than those in the simple hypertension group,systolic blood pressure and diastolic blood pressure were higher than those in the simple hypertension group(P<0.05).The levels of serum C1q,C8βand VEGF in the preeclampsia group were lower than those in the simple hypertension group,while levels of CFHRP5 and hs-CRP were higher than those in the simple hypertension group(P<0.05).The expression levels of serum C1q,C8βand VEGF in the non-severe group were higher than those in the severe group,while levels of CFHRP5 and hs-CRP were lower than those in the severe group(P<0.05).The results of ROC curve analysis showed that C1q,C8β,CFHRP5,hs-CRP,VEGF and their combined detection were of predictive value for preeclampsia(P<0.05).The levels of serum C1q,C8βand VEGF were negatively correlated with the severity of preeclampsia(P<0.05),while levels of CFHRP5 and hs-CRP were positively correlated with it(P<0.05).Conclusion The combined detection of serum C1q,C8β,CFHRP5,hs-CRP and VEGF has the value of predicting preeclampsia,and is correlated with the development of preeclampsia.
