妊娠期宫内感染对早产儿外周血T细胞亚群、免疫球蛋白及炎症因子影响

Effects of the intrauterine infection of premature infants on thelevels of their T-cell subsets,immunoglobulin,and inflammatory cytokines in peripheral blood

目的:探讨妊娠期宫内感染对早产儿外周血T细胞亚群、免疫球蛋白及炎症因子影响。方法:回顾性分析2010年1月-2020年1月于本院住院的早产儿900例资料,发生宫内感染122例(感染组),感染发生率为13.6%;未发生宫内感染778例,随机抽样150例纳入非感染组,监测早产儿出生24h及7d时外周血T细胞亚群、免疫球蛋白及炎症因子等指标水平。结果:感染组早产儿出生后24h及7d时,外周血T细胞亚群水平均高于非感染组,免疫球蛋白水平均低于非感染组,白细胞介素(IL)-1β、IL-6、IL-8和肿瘤坏死因子(TNF)-α水平均高于非感染组(均P<0.05)。结论:妊娠期宫内感染的早产儿出生后外周血T细胞亚群、免疫球蛋白及炎症因子水平均发生异常改变,提示临床应加以关注。

Objective:To investigate the effects of the intrauterine infection of premature infants on the levels of their T-cell subsets,immunoglobulin,and inflammatory cytokines in peripheral blood.Methods:900 premature infants from January 2010 to January 2020 were analyzed retrospectively.The incidence of intrauterine infection of the premature infants was 13.6%(122 cases with intrauterine infection)and 778 cases without intrauterine infection.122 cases with intrauterine infection were included in study group,150 cases from 778 cases without intrauterine infection were randomly selected in control group.The levels of peripheral blood T cell subsets,immunoglobulin,and inflammatory factors of the premature infants in the 24th hour and the 7th day after birth were monitored.Results:In the 24th hour and the 7th day after birth,the levels of T cell subsets,the interleukin(IL)-1β,IL-6,IL-8,and the tumor necrosis factor(TNF-α)in peripheral blood of the premature infants in the study group were significantly higher than those of the premature infants in the control group,but the level of immunoglobulin of the premature infants in the study group was significantly lower(all P<0.05).Conclusion:The levels of T-cell subsets,immunoglobulin,and inflammatory cytokines in peripheral blood of the premature infants with intrauterine infection during pregnancy are abnormal,which has certain clinical significance for assessing the prognosis of the premature infants with intrauterine infection.
