宣肺降浊清肠汤治疗稳定期支气管扩张患者的有效性评价

Effectiveness evaluation on Xuanfei Jiangzhuo Qingchang Decoction in treatment of stable bronchiectasis

目的 观察宣肺降浊清肠汤治疗稳定期支气管扩张(SBC)患者的有效性。方法 将我院60例SBC患者随机分为对照组与观察组各30例。观察组采用宣肺降浊清肠汤+氨溴索联合治疗,对照组采用氨溴索治疗,比较两组临床疗效,治疗前后生活质量、呼吸困难、24h痰量、急性加重次数、肺功能、氧化应激指标、不良反应发生情况。结果 治疗后,观察组总有效率显著高于对照组(P<0.05);两组患者圣乔治呼吸问卷(SGRQ)、mMRC得分、24h痰量、急性加重次数均下降,且观察组患者SGRQ、mMRC得分、24h痰量、急性加重次数低于对照组(P<0.05);两组患者用力肺活量(FVC)、第1s用力呼气容积(FEV1)均增加,且观察组患者FVC、FEV1均高于对照组(P<0.05);两组患者活性氧(ROS)值均下降,超氧化物歧化酶(SOD)值均增加,且观察组患者ROS值低于对照组,SOD值高于对照组(P<0.05);两组不良反应比较无明显差异(P>0.05)。结论 宣肺降浊清肠汤治疗SBC临床效果较好,能显著改善患者生活质量,缓解呼吸困难,降低咳痰量,恢复肺功能,且不良反应少,其机制可能与调节氧化应激水平有关。

Objective To observe the effectiveness of Xuanfei Jiangzhuo Qingchang Decoction in treatment of stable bronchiectasis(SBC). Methods Sixty SBC patients were randomly divided into control group and observation group with 30 cases each. Observation group was treated with Xuanfei Jiangzhuo Qingchang Decoction plus ambroxol,and control group was treated with ambroxol. The clinical effects,the quality of life before and after treatment,dyspnea,24 h sputum volume,number of acute exacerbations,lung function,indicators of oxidative stress and occurrence of adverse reactions of the two groups were compared. Results After treatment,the total effective rate of observation group was significantly higher than control group( P<0.05);the St. George’s Respiratory Questionnaire(SGRQ),mMRC score,24 h sputum volume,and the number of acute exacerbations were all decreased in both groups. mMRC score,24 h sputum volume,and number of acute exacerbations were lower than those of control group( P<0.05);the FVC and FEV1 of both groups increased,and observation group were higher in FVC and FEV1 than control group( P<0.05);ROS values decreased and SOD values increased in both groups,ROS values of observation group were lower than control group,and SOD values were higher than control group( P<0.05);there was no significant difference in adverse reactions between the two groups( P>0.05). Conclusion Xuanfei Jiangzhuo Qinchang Decoction has a good clinical effect in treatment of SBC,which could significantly improve the quality of life of patients,relieve dyspnea,reduce sputum production,restore lung function with fewer adverse reactions. The mechanism might be related to the regulation of oxidative stress.
