DBT联合DCE-MRI在致密型乳腺癌中应用价值研究 被引量:2

摘要 目的探究数字乳腺断层合成摄影(DBT)联合动态增强磁共振成像(DCE-MRI)在致密型乳腺癌中的应用价值。方法搜集2019年3月至2020年12月本院接诊的96例致密型乳腺癌患者作为致密型乳腺癌组研究对象,另外同期搜集52例非致密型乳腺癌作为非致密型乳腺癌组研究对象,两组均行DBT、DCE-MRI检查,并以组织病理学检验结果为金标准,分析诊断价值。结果与DBT、DCE-MRI单项诊断相比,DBT联合DCE-MRI对乳腺癌组织病理学的诊断价值较高,具有统计学差异(P<0.05)。致密型乳腺癌组在平均直径、边缘平整、内部强化占比与非致密型乳腺癌组存在显著差异(P<0.05)。受试者工作特征(ROC)曲线显示,DBT单项诊断致密型乳腺癌的曲线下面积(AUC)值、灵敏度、特异度、准确率分别为0.862、83.33%、77.28%、82.49%,DCE-MRI单项诊断致密型乳腺癌的AUC值、灵敏度、特异度、准确率分别为0.896、84.48%、75.37%、87.23%,DBT联合DCE-MRI诊断致密型乳腺癌的AUC值、灵敏度、特异度、准确度分别为0.944、95.23%、70.12%、93.35%,DBT联合DCE-MRI诊断价值明显高于单项诊断,具有统计学差异(P<0.05)。结论 DBT联合DCE-MRI可有效提高对致密型乳腺癌的诊断敏感度及准确率,值得在临床工作中推广使用。 Objective Explore the application value of digital breast tomosynthesis(DBT) combined with dynamic en-hanced magnetic resonance imaging(DCE-MRI) in dense breast cancer.Methods 96 patients with dense breast canceradmitted to our hospital from March 2019 to December 2020 were selected as the research object of the dense breast cancergroup,and 52 cases of non-dense breast cancer were selected as the research object of the non-dense breast cancer groupduring the same period. Both groups underwent digital breast tomosynthesis and dynamic enhanced magnetic resonance ima-ging examinations,and the histopathological examination results were used as the gold standard to analyze the diagnostic val-ue.Results Compared with the single diagnosis of DBT and DCE-MRI,DBT combined with DCE-MRI had a higher diag-nostic value for histopathology of breast cancer,with statistical differences(P< 0. 05). There were significant differences between the dense breast cancer group and the non-compact breast cancer group in the average diameter,flat edges,and theproportion of internal enhancement(P< 0. 05). The ROC curve showed that the AUC value,sensitivity,specificity,and ac-curacy of DBT for single diagnosis of dense breast cancer were 0. 862,83. 33%,77. 28%,and 82. 49%,respectively. TheAUC value,sensitivity,and sensitivity of DCE-MRI for single diagnosis of dense breast cancer were 0. 862,83. 33%,77.28%,and 82. 49%. The specificity and accuracy were 0. 896,84. 48%,75. 37%,and 87. 23%,respectively. The AUC val-ue,sensitivity,specificity,and accuracy of DBT combined with DCE-MRI in the diagnosis of dense breast cancer were 0.944,95. 23%,70. 12%,and 93. 35%,respectively,the diagnostic value of DBT combined with DCE-MRI was significantlyhigher than that of single diagnosis,with statistical difference(P< 0. 05).Conclusion DBT combined with DCE-MRIcan effectively improve the sensitivity and accuracy of the diagnosis of dense breast cancer,and it is worthy of promotion anduse in clinical work.
