IL-6、PCT、CRP在手足口病患儿中的表达及临床意义

Expression and Clinical Significance of IL-6,PCT and CRP in Children with HFMD

目的分析白介素-6(IL-6)、降钙素原(PCT)和C反应蛋白(CRP)在手足口病患儿中的表达情况和临床意义。方法将我院在2017年6月至2019年5月就诊的265例早期手足口病患儿为观察组,回顾临床资料分析,根据是否手足口病伴细菌感染分为单纯手足口病组(135例),手足口合并细菌感染组(130例)。对比分析两组血清白细胞(WBC)、IL-6、PCT和CRP水平含量和相关性。结果观察组WBC、CRP、IL-6和PCT含量均显著高于健康组(P<0.001),手足口合并细菌感染组WBC、CRP、IL-6和PCT水平均显著高于单纯手足口病组(P<0.001);与CRP、IL-6和WBC相比,PCT的AUC值和特异度有统计学意义(P<0.05),与PCT、CRP和WBC相比,IL-6的灵敏度有统计学意义(P<0.05)。结论在手足口病的分型诊断中,PCT诊断价值最大,PCT联合IL-6诊断手足口病其诊断价值最高,特异度和灵敏度最高。

Objective To analyze the expression and clinical significance of interleukin-6(IL-6),procalcitonin(PCT)and C-reactive protein(CRP)in children with hand, foot and mouth disease(HFMD).Methods A total of 265 children with early hand, foot and mouth disease who were treated in our hospital from June 2017 to May 2019 were enrolled as the observation group.The clinical data were retrospectively analyzed.According to the presence or absence of HFMD with bacterial infection, they were divided into simple HFMD group(135)and combined HFMD infection group(130 cases).The levels and correlations of serum leukocyte(WBC),IL-6,PCT and CRP levels were compared between the two groups.Results The WBC,CRP,IL-6 and PCT levels in the observation group were significantly higher than those in the healthy group(P<0.001).The WBC,CRP,IL-6 and PCT levels in the HFMD infection group were significantly higher than those in simple HFMD group(P<0.001).Compared with CRP,IL-6 and WBC,the AUC value and specificity of PCT were statistically significant(P<0.05),and the sensitivity of IL-6 was statistically significant in relative to PCT,CRP and WBC(P<0.05).Conclusion In the differential diagnosis of hand, foot and mouth disease, PCT has the greatest diagnostic value.PCT combined with IL-6 has the highest diagnostic value, specificity and sensitivity for diagnosis of hand, foot and mouth disease.
