通乳丹、子午流注开穴联合低频脉冲电刺激治疗产后缺乳临床观察

Clinical Observation of Tongru Dan and Ziwuliuzhu Acupuncture Combined with Low-Frequency Pulse Electrical Stimulation in the Treatment of Postpartum Hypogalactia

目的探讨通乳丹口服、子午流注开穴联合低频脉冲电刺激治疗产后缺乳的临床疗效及安全性。方法选取医院2019年1月至2021年1月收治的产后缺乳患者103例,按随机数字表法分为观察组(52例)和对照组(51例)。两组患者均予低频脉冲电刺激治疗,观察组患者加用通乳丹口服、子午流注开穴治疗。两组患者均以14 d为1个疗程,连续治疗3个疗程。结果观察组总有效率为90.38%,显著高于对照组的74.51%(P<0.05);观察组患者治疗第2,4,6周泌乳量均显著高于对照组(P<0.05);治疗后,观察组患者泌乳素、雌二醇水平均显著高于对照组,血清孕酮水平显著低于对照组(P<0.05);两组患者的乳汁涩少、神疲乏力、食欲不振、面色无华等中医证候积分均显著低于治疗前,且观察组显著低于对照组(P<0.05)。观察组和对照组治疗期间不良反应发生率相当(15.38%比9.80%,P>0.05)。结论通乳丹口服、子午流注开穴联合低频脉冲电刺激治疗产后缺乳的疗效显著,可提高患者的泌乳量,改善中医证候积分和雌激素水平,且治疗安全性良好。

Objective To investigate the clinical efficacy and safety of Tongru Dan,Ziwuliuzhu acupuncture combined with low-frequency pulse electrical stimulation in the treatment of postpartum hypogalactia.Methods A total of 103 patients with postpartum hypogalactia admitted to the hospital from January 2019 to January 2021 were selected and randomly divided into the observation group(52 cases)and the control group(51 cases).The patients in the two groups were treated with low-frequency pulse electrical stimulation,on this basis,the patients in the observation group were treated with Tongru Dan and Ziwuliuzhu acupuncture.Both groups were treated for three consecutive courses with 14 d as a course of treatment.Results The total effective rate of the observation group was 90.38%,which was significantly higher than 74.51%of the control group(P<0.05).The lactation volume in the observation group at the second,fourth and sixth week of treatment was significantly higher than that of the control group(P<0.05).After treatment,the levels of prolactin and estradiol in the observation group were significantly higher than those in the control group,while the level of serum progesterone in the observation group was significantly lower than that in the control group(P<0.05).After treatment,the scores of traditional Chinese medicine(TCM)syndromes including less astringency of milk,mental fatigue,loss of appetite and pale complexion in the two groups were significantly lower than those before treatment,and the above points in the observation group were significantly lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was similar to that in the control group(15.38%vs.9.80%,P>0.05).Conclusion Tongru Dan and Ziwuliuzhu acupuncture combined with low-frequency pulse electrical stimulation are effective and safe in the treatment of postpartum hypogalactia,which can improve the lactation volume,improve the score of TCM syndromes and the level of estrogen.
