摘要
目的分析生化免疫检验中化学发光免疫测定技术的应用价值。方法226例疑似甲状腺肿瘤患者,根据检验方法不同分为观察组(114例)和对照组(112例)。对照组采取常规放射免疫分析法检测,观察组采用化学发光免疫法检测。以病理检查结果为金标准,比较两组检测敏感性、特异性、准确率以及球蛋白、甲状腺球蛋白、游离三碘甲状腺原氨酸、游离甲状腺素、促甲状腺激素水平。结果经病理诊断,观察组甲状腺肿瘤阳性100例,阴性14例;对照组甲状腺肿瘤阳性98例,阴性14例。观察组检测敏感性97.0%、特异性92.9%、准确率96.5%均高于对照组的81.6%、57.1%、78.6%,差异均具有统计学意义(P<0.05)。观察组球蛋白(14.0±0.5)个、甲状腺球蛋白(689.5±75.8)ng/ml、游离三碘甲状腺原氨酸(37.6±21.5)pmol/L、游离甲状腺素(84.6±41.3)pmol/L、促甲状腺激素(3.6±1.3)mIU/L均高于对照组的(12.0±0.3)个、(632.5±60.7)ng/ml、(25.3±18.2)pmol/L、(73.5±38.6)pmol/L、(1.2±0.8)mIU/L,差异均具有统计学意义(P<0.05)。结论化学发光免疫法在生化免疫检验中的应用价值要明显高于放射免疫分析法,能够提高临床检验结果的敏感性、特异性与准确率,进一步提高各项生化指标的检验精度,值得推广使用。
Objective To analyze the application value of chemiluminescence immunoassay in biochemical immunoassay.Methods A total of 226 patients with suspected thyroid tumors were divided into observation group(114 cases)and control group(112 cases)according to the different testing methods.The control group was tested by conventional radioimmunoassay,and the observation group was tested by chemiluminescence immunoassay.The pathological results were used as the gold standard to compare the sensitivity,specificity,and accuracy of the two testing methods as well as the levels of globulin,thyroglobulin,free triiodothyronine,free thyroxine,and thyroid stimulating hormone.Results After pathological diagnosis,there were 100 positive patients and 14 negative patients in the observation group;98 positive patients and 14 negative pationts in the control group.The sensitivity 97.0%,specificity 92.9%and accuracy 96.5%in the observation group were higher than 81.6%,57.1%and 78.6%in the control group,and the differences were statistically significant(P<0.05).In the observation group,the globulin was(14.0±0.5),the thyroglobulin was(689.5±75.8)ng/ml,the free triiodothyronine was(37.6±21.5)pmol/L,the free thyroxine was(84.6±41.3)pmol/L,and the TSH was(3.6±1.3)mIU/L,which were higher than(12.0±0.3),(632.5±60.7)ng/ml,(25.3±18.2)pmol/L,and(73.5±38.6)pmol/L and(1.2±0.8)mIU/L in the control group,and the difference was statistically significant(P<0.05).Conclusion The application value of chemiluminescence immunoassay in biochemical immunoassay is significantly higher than that of radioimmunoassay,which can improve the sensitivity,specificity and accuracy of clinical test results,and further improve the test accuracy of various biochemical indicators,so it is worthy of popularization and application.
