子宫内膜异位症患者血清甲胎蛋白、趋化因子受体5表达水平及其临床应用价值

The expression levels and clinical significance of serum AFP and CCR5 in patients with endometriosis

目的探讨甲胎蛋白(AFP)、趋化因子受体5(CCR5)在子宫内膜异位症(EMs)患者血清中的表达情况及其临床应用价值。方法选取2018年6月至2020年6月遵化市人民医院收治的70例EMs患者作为观察组。另选取同期65例健康体检女性作为对照组。采用电化学发光免疫分析法检测两组血清AFP表达水平,采用酶联免疫吸附试验法检测两组血清CCR5表达水平;比较两组的血清AFP、CCR5表达水平;采用受试者工作特征(ROC)曲线评价血清AFP、CCR5表达水平对于EMs的临床诊断价值;采用Logistic回归分析EMs的影响因素。结果观察组血清AFP、CCR5表达水平显著高于对照组,差异具有统计学意义(P<0.05);临床分期Ⅲ~Ⅳ期的血清AFP、CCR5表达水平高于临床分期I~Ⅱ期,差异具有统计学意义(P<0.05);ROC曲线显示,血清AFP、CCR5表达水平对EMs诊断的曲线下面积分别为0.922(灵敏度88.6%,特异度89.2%,截断值为2.572ng/mL)、0.856(灵敏度60.0%,特异度69.0%,截断值7.466ng/mL);Logistic回归分析显示,AFP、CCR5均是EMs的独立危险因素。结论AFP、CCR5在EMs患者血清中呈异常高表达,血清AFP、CCR5检测对于EMs的临床诊断具有重要参考价值。

Objective To investigate the expression of alpha-fetoprotein(AFP)and C-C chemokine receptoer type 5(CCR5)in the serum of patients with endometriosis(EMs)and their clinical significance.Methods Seventy EMs patients admitted to People′s Hospital of Zunhua County from June 2018 to June 2020 were selected as the observation group.Another 65 healthy women were selected as the control group.The expression level of serum AFP was detected by electrochemiluminescence immunoassay,and the expression level of serum CCR5 was detected by enzyme-linked immunosorbent assay.The expression levels of serum AFP and CCR5 were compared between the two groups.Receiver operating characteristic(ROC)curve was used to evaluate the clinical diagnostic value of serum AFP and CCR5 expression levels for EMs.The influencing factors of EMs were analyzed by Logistic regression.Results The expression levels of serum AFP and CCR5 in the observation group were significantly higher than those in the control group(P<0.05).The expression levels of serum AFP and CCR5 in clinical stageⅢtoⅣwere higher than those in clinical stageⅠtoⅡ(P<0.05).ROC curve showed that the area under the curve of serum AFP and CCR5 expression for EMs diagnosis was 0.922(sensitivity 88.6%,specificity 89.2%,cut-off value 2.572ng/mL)and 0.856(sensitivity 60.0%,specificity 69.0%,cut-off value 7.466ng/mL),respectively.Logistic regression analysis showed that AFP and CCR5 were independent risk factors of EMs.Conclusions AFP and CCR5 are abnormally highly expressed in the serum of patients with EMs.The detection of serum AFP and CCR5 has important reference value for the clinical diagnosis of EMs.
