摘要
目的观察尖锐湿疣(CA)患者经二氧化碳(CO_(2))激光联合抗人乳头瘤病毒(HPV)生物蛋白敷料治疗后复发的相关因素。方法选取2019年4月至2021年4月北京市门头沟区医院皮肤性病科门诊诊治的90例CA患者作为研究对象。全部患者均接受CO_(2)激光联合抗HPV生物蛋白敷料治疗,并随访3个月;根据随访期间复发情况将患者分为复发组与未复发组;设计患者基线资料调查表,比较两组基线资料,分析CA患者经CO_(2)激光联合抗HPV生物蛋白敷料治疗后复发的影响因素。结果90例CA患者经CO_(2)激光联合抗HPV生物蛋白敷料治疗后复发33例,复发率为36.67%;经Logistic回归分析,结果显示,高危型HPV感染、CD8^(+)高及雌二醇(E_(2))、孕酮(P)、血管内皮生长因子(VEGF)水平高是CA患者经CO_(2)激光联合抗HPV生物蛋白敷料治疗后复发的危险因素(OR>1,P<0.05),CD4^(+)高是其保护因素(OR<1,P<0.05)。结论CA患者经CO_(2)激光联合抗HPV生物蛋白敷料治疗后复发可能受高危型HPV感染、CD4^(+)、CD8^(+)及E_(2)、P、VEGF水平的影响。
Objective To observe the related factors of recurrence in patients with condyloma acuminatum(CA)treated with carbon dioxide(CO_(2))laser combined with anti-human papilloma virus(HPV)biological protein dressing.Methods 90 patients with CA treated in Department of Dermatology and Veneredogy Clinic of Beijing Mentougou District Hospital from April 2019 to April 2021 were selected as the research subjects,all patients were treated with CO_(2)laser combined with anti-HPV biological protein dressing,and were followed up for 3 months.According to the recurrence during follow-up,the patients were divided into recurrence group and non-recurrence group.Designed the patient baseline data questionnaire,the baseline data of patients in the two groups were compared,the influencing factors of recurrence in patients with CA after CO_(2)laser combined with anti-HPV biological protein dressing.Results Among 90 CA patients treated with CO_(2)laser combined with anti-HPV biological protein dressing,33 cases recurred,and the recurrence rate was 36.67%(33/90).Logistic regression showed that high-risk HPV infection,high CD8^(+) value and high levels of estradiol(E_(2)),progesterone(P),vascular endothelial growth factor(VEGF)were the risk factors for recurrence of CA patients after carbon dioxide laser combined with anti-HPV biological protein dressing(OR>1,P<0.05),the high CD4^(+) value was the protective factor(OR<1,P<0.05).Conclusions The recurrence of CA patients after CO_(2)laser combined with anti-HPV biological protein dressing may be affected by high-risk HPV infection,CD4^(+),CD8^(+) and the levels of E_(2),P and VEGF.
