炎症肠道菌群-体育锻炼与骨关节炎相关性研究进展

摘要 骨关节炎(osteoarthritis,OA)被认为是最常见的关节疾病之一,随着全球老年化进程的加剧和饮食习惯以及人们生活方式的改变,患病率逐年攀高。而目前,肠道菌群在OA发病机制中的作用尚不清楚,肠道生态失调的肠道菌群(GM)生物多样性和功能的改变可能参与炎性反应的发展,而炎性反应是OA发展的主要危险因素之一。众所周知,体育锻炼在预防和治疗包括OA在内的一些慢性疾病中发挥着重要作用,并且被一些指南推荐作为治疗该疾病的一线干预手段。有研究表明,体育锻炼可以调节肠道菌群的组成,提高肠黏膜免疫,增加拟杆菌门厚壁菌门的比例,改变胆红酸的分布,并提高短链脂肪酸(SCFAs)的分泌,此外,已有研究显示,低强度的体育运动有助于降低患胃肠道疾病的风险,这也证实了之前提出的体育锻炼与肠道菌群之间存在严格相关性的假设。然而,目前针对该领域的相关临床试验仍然缺乏。因此,文章总结与之相关的最新文献,支持GM与OA之间相关性的假设,并强调体育锻炼结合适当的饮食和益生菌补充在平衡肠道微生物生态失调中的作用。 Osteoarthritis(OA)is considered to be one of the most common joint diseases.With the aggravation of the global aging process and the change of dietary habits and people’s lifestyle,the prevalence of OA is increasing year by year.However,at present,the role of gut microflora(GM)in the pathogenesis of OA is still unclear.Changes in the biodiversity and function of intestinal dysbiosis GM may be involved in the development of inflammatory response,which is one of the major risk factors for the development of OA.It is well known that physical activity plays an important role in the prevention and treatment of a number of chronic diseases,including OA,and is recommended by some guidelines as a first-line intervention in the treatment of this disease.Studies have shown that exercise can adjust the composition of GM,improving intestinal mucosal immunity,increase the proportion of bacteroidetes door thick wall fungus door,change the distribution of red bile acid,and improve the production of short chain fatty acids(SCFAs),in addition,the existing studies show that low intensity sports helps reduce the risk of gastrointestinal disease,this also confirmed before physical exercise and are put forward.It is assumed that there is a strict correlation between the GM.However,there is still a lack of clinical trials in this field.In this review,the authors aim to summarize the latest literature on GM and OA,to support the hypothesis of an association between GM and OA,and to highlight the role of physical exercise in combination with appropriate diet and probiotic supplementation in balancing intestinal microbial imbalance.
