亲水性银离子敷料联合外用重组人碱性成纤维细胞生长因子治疗Ⅱ°烧伤的临床疗效观察

Observation of clinical efficacy of hydrophilic silver ion dressing combined with external application of recombinant human basic fibroblast growth factor in the treatment ofⅡ°burns

目的观察亲水性银离子敷料联合外用重组人碱性成纤维细胞生长因子治疗Ⅱ°烧伤的效果。方法选取2020年2月至2021年8月我院收治的96例Ⅱ°烧伤患者当作研究对象,按随机抽签法分成2组,各48例,单一组给予外用重组人碱性成纤维细胞生长因子,联合组在单一组基础上加用亲水性银离子敷料。比较2组疗效、恢复情况(愈合时间、溶痂时间、住院时间)、换药疼痛程度[视觉模拟量表(VAS)评分]、炎性因子水平[白介素-1β(IL-1β)、白介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)]、生长因子水平[血管内皮生长因子(VEGF)、转化生长因子β1(TGF-β1)、碱性成纤维细胞因子(bFGF)]。结果联合组治疗总有效率95.83%较单一组79.17%高,愈合时间、溶痂时间、住院时间均较单一组短,换药疼痛程度优于单一组(P<0.05);2组治疗3周后TNF-α、IL-1β、IL-6、CRP水平均下降,且联合组低于单一组(P<0.05);2组治疗3周后VEGF、TGF-β1、bFGF水平均上升,且联合组高于单一组(P<0.05)。结论亲水性银离子敷料联合外用重组人碱性成纤维细胞生长因子治疗Ⅱ°烧伤疗效显著,能促进创面愈合,降低换药疼痛程度,减轻炎症反应,提高生长因子水平,缩短康复进程。

Objective To observe the effect of hydrophilic silver ion dressing combined with external application of recombinant human basic fibroblast growth factor in the treatment of second-degree burns.Methods A total of 96 patients withⅡ°burns who were treated in our hospital from February 2020 to August 2021 were selected as the research subjects,and divided into 2 groups according to random lottery,with 48 cases in each group.factor,the combined group was supplemented with a hydrophilic silver ion dressing on the basis of a single group.The curative effect,recovery(healing time,scab-dissolving time,hospitalization time),dressing change pain degree[visual analog scale(VAS)score],inflammatory factor levels[interleukin-1β(IL-1β),interleukin-6(IL-6),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)],growth factor levels[vascular endothelial growth factor(VEGF),transforming growth factor beta 1(TGF-β1),basic fibroblast factor(bFGF)].Results The total effective rate of the combined group was 95.83%higher than that of the single group(79.17%),the healing time,scab-dissolving time and hospital stay were shorter than those of the single group,and the pain degree of dressing change was better than that of the single group(P<0.05).The two groups were treated for 3 weeks.After 3 weeks of treatment,the levels of TNF-α,IL-1β,IL-6 and CRP all decreased,and the combined group was lower than that of the single group(P<0.05).The group was higher than the single group(P<0.05).Conclusion Hydrophilic silver ion dressing combined with external application of recombinant human basic fibroblast growth factor has a significant curative effect in the treatment ofⅡ°burns,can promote wound healing,reduce the pain degree of dressing change,reduce inflammatory response,increase the level of growth factor,and shorten the recovery process.
