中医药治疗慢性萎缩性胃炎肿瘤免疫细胞因子机制研究进展

Research Progress of Tumor Immune Cytokine Mechanism in the Treatment of Chronic Atrophic Gastritis with Traditional Chinese Medicine

慢性萎缩性胃炎(CAG)是胃癌前疾病,相较于普通慢性胃炎,其具有更高的癌变几率,为降低胃癌的发生率,有必要在癌前阶段及早治疗。但是目前西医缺乏有效逆转萎缩的药物,以根除幽门螺旋杆菌(HP)、对症支持治疗为主,严重影响患者的生存质量。而中医药治疗CAG具有显著的临床疗效,具有延缓病程、改善症状、逆转萎缩等优点,肿瘤相关免疫细胞因子与CAG的"炎-癌转化"密切相关,可促进炎症、损害局部组织细胞、诱导细胞异常增殖及恶变,最终导致胃癌的发生,诸多研究表明中医药可通过影响肿瘤相关免疫细胞因子的表达,从而达到调节免疫、控制炎症、逆转"炎-癌转化"的作用。该文拟从中医药调控白细胞介素、干扰素、肿瘤坏死因子、生长因子等4个角度,系统总结近年来中医药防治CAG"炎-癌转化"的机制研究进展,为阐明中医药治疗CAG的作用机制提供思路,进而促进中医药治疗CAG的推广与应用,提升CAG的临床疗效。

Chronic atrophic gastritis(CAG)is a precancerous disease of the stomach. Compared with ordinary chronic gastritis,it has a higher chance of canceration. In order to reduce the incidence of gastric cancer,it is necessary to treat it early in the precancerous stage. However,currently Western medicine lacks drugs that can effectively reverse atrophy,focusing on eradication of HP and symptomatic and supportive treatment,which seriously affects the quality of life of patients. The Chinese medicine treatment of CAG has significant clinical effects,and has the advantages of delaying the course of the disease,improving symptoms,and reversing atrophy. Tumor-related immune cytokines are closely related to the “inflammation-cancer transformation”of CAG,which can promote inflammation,damage local tissue and cells,induce abnormal cell proliferation and malignant transformation,and ultimately lead to the occurrence of gastric cancer. A lot of studies have shown that traditional Chinese medicine can regulate immunity,control inflammation,and reverse“inflammation-cancer transformation”by affecting the expression of tumor-related immune cytokines. This article intends to systematically summarize the research progress of the mechanism of traditional Chinese medicine in preventing and treating CAG“inflammation-cancer transformation”in recent years from the four perspectives of Chinese medicine regulating interleukin,interferon,tumor necrosis factor,growth factor,etc.,in order to clarify the role of traditional Chinese medicine in treating CAG. So as to promote the promotion and application of traditional Chinese medicine to treat CAG and enhance the clinical efficacy of CAG.
