腹腔镜手术在异位妊娠中的疗效及对卵巢功能的影响研究

Study on the Effect of Laparoscopic Surgery on Patients with Ectopic Pregnancy and Influence on Ovarian Function

目的探究及分析腹腔镜手术在异位妊娠中的疗效及对卵巢功能的影响。方法根据随机的原则将2018年5月—2021年5月80例异位妊娠患者分为对照组和观察组,每组40例。比较两组的术中出血量、手术时间、排气时间、住院时间、输卵管通畅率、手术前后的卵巢功能相关指标(血清生殖激素及窦卵泡数量、卵巢体积)。结果观察组的术中出血量显著少于对照组,手术时间、排气时间及住院时间显著短/早于对照组,输卵管通畅率显著高于对照组,差异有统计学意义(P<0.05),术前两组的血清生殖激素比较,差异无统计学意义(P>0.05),术后1、2个月及3个月两组的血清E_(2)先降后升,血清FSH及LH则先升后降,窦卵泡数量及卵巢体积持续升高,观察组的血清E_(2)、窦卵泡数量及卵巢体积显著高于对照组,血清FSH及LH显著低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜手术在异位妊娠中的疗效较好,且对卵巢功能的不良影响较小,因此在异位妊娠患者中的应用价值较高。

Objective To investigate and analyze the effect of laparoscopic surgery in patients with ectopic pregnancy and influence on ovarian function.Methods A total of 80 patients with ectopic pregnancy from May 2018 to May 2021 were divided into control group and observation group with 40 cases in each group by the random principle.Then the intraoperative blood loss,operation time,exhaust time,hospitalization time,patency rate of fallopian tube,ovarian function related indexes(serum reproductive hormones and antral follicles count,ovarian volume)before and after the surgery of two groups were compared.Results The intraoperative blood loss of observation group was significantly less than that of control group,the operation time,exhaust time and hospitalization time were significantly shorter/earlier than those of control group,the patency rate of fallopian tube was significantly higher than that of control group,the differences were all statistically significant(P<0.05).The serum reproductive hormones of two groups were compared,there were no statistically significant differences(P>0.05).The serum E_(2) of two groups at 1st,2nd and 3rd after the surgery decreased first and increased afterward,the serum FSH and LH increased first and decreased afterward,antral follicles count and ovarian volume continuously increased,serum E_(2),antral follicles count and ovarian volume of observation group were higher than those of control group,serum FSH and LH were significantly lower than those of control group,the differences were all statistically significant(P<0.05).Conclusion The effect of laparoscopic surgery in patients with ectopic pregnancy is better,and its has less bad influence for ovarian function,so its application value in the patients with ectopic pregnancy is higher.
