彩色多普勒超声联合血浆D-二聚体诊断下肢静脉血栓的临床价值分析

Analysis on the Clinical Value of Color Doppler Ultrasound Combined with Plasma D-Dimer in the Diagnosis of Lower Limb Venous Thrombosis

目的分析彩色多普勒超声联合血浆D-二聚体(D-D)诊断下肢静脉血栓的临床价值。方法选取2018年3月至2020年3月我院收治的95例疑似下肢静脉血栓患者,根据下肢静脉造影结果分为血栓组(65例,发生下肢静脉血栓)与无血栓组(30例,未发生下肢静脉血栓)。两组均使用彩色多普勒超声与D-D诊断,记录两组的D-D水平、静脉内径,比较诊断效能。结果血栓组的D-D水平高于无血栓组(P<0.05)。血栓组大隐静脉(GSV)、股浅静脉(SFV)、总静脉(CFV)、股深静脉(PFV)内径高于无血栓组(P<0.05)。彩色多普勒超声联合D-D诊断敏感度、阴性预测值高于彩色多普勒超声、D-D单一诊断(P<0.05)。结论彩色多普勒超声联合血浆D-D诊断下肢静脉血栓的效果较好,可明显提高诊断效能,临床应用价值较高。

Objective To analyze the clinical value of color Doppler ultrasound combined with plasma D-dimer(D-D)in the diagnosis of lower limb venous thrombosis.Methods 95 patients with suspected lower limb venous thrombosis admitted to our hospital from March 2018 to March 2020 were selected and divided into thrombosis group(65 cases,lower limb venous thrombosis)and non-thrombosis group(30 cases,without lower limb venous thrombosis)according to the results of lower limb venography.Both groups were diagnosed by color Doppler ultrasound and D-D,the D-D levels and venous diameter of the two gorups were recorded,and the diagnostic efficiency was compared.Results The D-D level of the thrombosis group was higher than that of the non-thrombosis group(P<0.05).The diameters of great saphenous vein(GSV),superficial femoral vein(SFV),common femoral vein(CFV)and deep femoral vein(PFV)in the thrombosis group were higher than those in the non-thrombosis group(P<0.05).The sensitivity and negative predictive value of color Doppler ultrasound combined with D-D diagnosis were higher than those of single diagnosis of color Doppler ultrasound and D-D(P<0.05).Conclusions Color Doppler ultrasound combined with plasma D-D has better effect in the diagnosis of lower limb venous thrombosis,which can significantly improve the diagnostic efficiency,with higher clinical application value.
