MRI及CT诊断肝硬化合并肝癌的影像学特点及诊断效能分析

Analysis on the Imaging Features and Diagnostic Efficiency of MRI and CT in the Diagnosis of Liver Cirrhosis Complicated with Liver Cancer

目的分析MRI与CT诊断肝硬化合并肝癌的影像学特点及诊断效能。方法选择2015年6月至2020年7月我院收治的50例疑似肝硬化合并肝癌患者,均进行MRI、CT检查,以最终病理诊断为依据,比较两种检查方法的诊断准确率、灵敏度、特异度。结果MRI检查中肝硬化合并肝癌患者病灶多表现为肿块边界清晰,T1WI序列呈低信号,T2WI呈高信号,动脉期、门静脉、延迟期高信号;CT检查中肝硬化合并肝癌患者病灶多表现为肿块边缘清晰,平扫低密度,动脉期高密度,门静脉期、延迟期低密度。MRI准确率、灵敏度显著高于CT(P<0.05)。结论MRI较CT诊断肝硬化合并肝癌更具优势,临床诊断时可优先考虑。

Objective To analyze the imaging features and diagnostic efficiency of MRI and CT in the diagnosis of liver cirrhosis complicated with liver cancer.Methods 50 patients with suspected liver cirrhosis complicated with liver cancer admitted to our hospital from June 2015 to July 2020 were selected and given MRI and CT.Based on the final pathological diagnosis,the diagnostic accuracy,sensitivity and specificity of the two examination methods were compared.Results In MRI,the lesions in patients with liver cirrhosis complicated with liver cancer mostly showed clear tumor boundaries,low signal on T1WI,high signal on T2WI,and high signal in arterial phase,portal vein,and delayed phase.In CT,the lesions in patients with liver cirrhosis complicated with liver cancer mostly manifested as tumors with clear edges,low density in plain scan,high density in arterial phase,and low density in portal vein and delayed phase.The accuracy and sensitivity of MRI were significantly higher than those of CT(P<0.05).Conclusions MRI has more advantages than CT in the diagnosis of liver cirrhosis complicated with liver cancer,which can be given priority in clinical diagnosis.
