摘要
目的 分析CysC、 Hcy、 NT-proBNP在心衰合并肾损伤患者临床诊断中的检测意义。方法 选取2018年8月至2020年8月我院收治的90例心衰患者,根据估算肾小球滤过率(eGFR)不同分为单纯心衰组(n=25)、心衰合并轻度肾损伤组(n=35)、心衰合并中重度肾损伤组(n=30),检测三组患者的CysC、 Hcy、 NT-proBNP水平,并对比单项检测与联合检测的诊断效能,分析心衰合并肾损伤患者死亡的相关因素。结果 心衰合并中重度肾损伤组的CysC、 Hcy、 NT-proBNP水平均高于心衰合并轻度肾损伤组、单纯心衰组,且心衰合并轻度肾损伤组均高于单纯心衰组(P <0.05)。联合检测的敏感度、特异度高于单项检测(P<0.05)。心衰合并肾损伤患者死亡的相关因素包括心功能分级、 CysC、 Hcy及NT-proBNP (OR>1, P<0.05)。结论 CysC、 Hcy、 NT-proBNP水平可有效反映心衰合并肾损伤的发病与进展情况,为临床治疗与预后判断提供参考依据。
Objective To analyze the Cys C, Hcy and NT-proBNP in the clinical diagnosis of patients with heart failure complicated with renal injury. Methods 90 patients with heart failure admitted to our hospital from August 2018 to August 2020 were divided into simple heart failure group(n=25), heart failure complicated with mild renal injury group(n=35) and heart failure complicated with moderate-severe renal injury group(n=30) according to different estimated glomerular filtration rates(eGFR). The Cys C, Hcy and NT-proBNP levels of the three groups were detected. The diagnostic efficiency of single detection and combined detection was compared.The related factors of death in patients with heart failure complicated with renal injury were analyzed. Results The Cys C, Hcy and NT-proBNP levels of the heart failure complicated with moderate-severe renal injury group were higher than those of the heart failure complicated with mild renal injury group and simple heart failure group, and the mild renal injury group was higher than simple heart failure group(P<0.05). The sensitivity and specificity of combined detection were higher than those of single detection(P<0.05). The related factors of death in patients with heart failure complicated with renal injury included cardiac function grade, Cys C, Hcy and NT-proBNP(OR>1, P<0.05). Conclusions Cys C, Hcy and NT-proBNP levels can effectively reflect the attack and progress of heart failure complicated with renal injury, and provide reference for clinical treatment and prognosis.
