猪苓汤治疗前列腺增生症水热互结型尿潴留临床观察

目的:观察猪苓汤对前列腺增生症水热互结型尿潴留对排尿时间及自行排尿能力恢复的影响。方法:70例根据随机数字表法分成对照组和研究组各35例。两组均予相同的基础治疗,对照组予溴新斯的明口服,观察组予猪苓汤加减煎服。结果:观察组总有效率高于对照组(P<0.05)。治疗后两组I-PSS评分,尿动力学参数最大尿流量(Q-max)、最大尿意膀胱容量(VMCC)、残余尿量(RUV)降低(P<0.05),逼尿肌受损、膀胱低顺应性和膀胱感觉缺失发生率降低(P<0.05),排尿无力、小腹坠胀和倦怠乏力症状评分降低(P<0.05)。与对照组比较,研究组Q-max更高(P<0.05),VMCC、RUV更低(P<0.01),逼尿肌受损、膀胱低顺应性和膀胱感觉缺失发生率更低(P<0.05),自行排尿、完全通畅排尿、排便、进食时间更短(P<0.01)。结论:猪苓汤加减治疗前列腺增生症水热互结型尿潴留可促进膀胱尿液排出,减少残余尿量,缩短自行排尿时间,提高自主排尿恢复能力。

Objective:To observe the effect of Zhuling Decoction on the recovery of urination time and spontaneous urination ability in patients with benign prostatic hyperplasia with hydrothermal retention type. Methods: According to the random number table method, 70 cases were divided into the control group and the research group, 35 cases in each group. The two groups were given the same basic treatment, and the control group were given oral neostigmine bromide while the observation group were given modified Zhuling Decoction. Results: The total effective rate of the observation group was higher than that of the control group(P<0.05). After treatment, the I-PSS scores, urodynamic parameters of maximum urine flow(Q-max), maximum urinary bladder capacity(VMCC), and residual urine volume(RUV) in the two groups decreased(P<0.05), while detrusor damage, low bladder compliance and bladder sensory loss decreased(P<0.05), and the symptom scores of micturition weakness, lower abdominal distention and lassitude were decreased(P<0.05). Compared with the control group, the study group had higher Q-max(P<0.05),lower VMCC and RUV(P<0.01), and lower incidence of detrusor damage, low bladder compliance and bladder sensory loss(P<0.01).The spontaneous urination, completely unobstructed urination, defecation, and eating time of the observation group were shorter than those of the control group(P<0.01). Conclusion: Zhuling Decoction can promote the excretion of urine from the bladder, reduce the residual urine volume, shorten the time of spontaneous urination, and improve the recovery ability of spontaneous urination in the treatment of benign prostatic hyperplasia of hydrothermal urinary retention type.
