三维超声Omniview与Render成像在宫腔粘连诊断中的应用对比

Comparison of three-dimensional ultrasound omniview and render imaging in the diagnosis of intrauterine adhesion

摘要 目的探讨三维超声(3D-TVS)自由解剖成像(Omniview)与传统容积成像(Render)对宫腔粘连(IUA)的诊断价值.方法选取宜春市妇幼保健院收治的疑似IUA患者60例,采用VOLUSON-S10彩色多普勒超声诊断仪对其进行阴道超声检查,分别以Omniview及Render模式进行宫腔成像;以宫腔镜(HS)检查结果作为金标准,比较两种成像模式诊断IUA的效能,总结IUA的三维超声图像特征.结果60例患者经HS检查确诊IUA 53例(88.33%),无IUA者7例(11.67%).采用Render模式诊断IUA患者45例,其中正确诊断41例(91.11%),无IUA者15例,其中正确诊断3例(20.00%);采用Omniview模式诊断IUA患者54例,其中正确诊断52例(96.30%),无IUA者6例,其中正确诊断5例(83.33%);以HS诊断结果为金标准,Omniview成像模式诊断IUA的灵敏度、特异度、准确率、阴性预测值、Kappa值、Youden指数及ROC曲线下面积(AUC)分别为98.11%、71.43%、95.00%、83.33%、0.741、0.695、0.848,均明显高于Render模式;Omniview模式诊断IUA阴性、轻度粘连及重度粘连的准确率明显高于Render模式(P<0.05).结论经阴道三维超声Omniview模式诊断IUA具有良好的诊断符合率,且对不同粘连程度IUA分型诊断准确率高于Render模式,可作为临床诊治IUA的首选辅助手段.
