检测血浆BNP、Fib及D-D水平在慢性阻塞性肺疾病合并肺栓塞中的临床意义

Clinical Significance of Plasma BNP,FIB and D-D Levels in Patients with Chronic Obstructive Pulmonary Disease Complicated with Pulmonary Embolism

目的:探讨血浆B型脑钠肽(BNP)、纤维蛋白原(Fib)及D二聚体(D-D)联合检测在诊断慢性阻塞性肺疾病(COPD)合并肺栓塞(PE)及判断预后方面的应用价值。方法:选取406例COPD患者为研究对象,根据有无合并肺栓塞分组,324例单纯COPD患者为COPD组,82例COPD合并PE患者为COPD合并PE组;根据PE危险程度将COPD合并PE患者分为三组:高危组(9例)、中危组(40例)、低危组(33例);根据预后将COPD合并PE患者分为两组:存活组(76例)、死亡组(6例)。记录患者入院时外周静脉血检测BNP、Fib及D-D的水平。结果:在血浆BNP、Fib、D-D水平方面,COPD合并PE组高于COPD组(P<0.05);高危组高于中危组和低危组(P<0.05);中危组高于低危组(P<0.05);死亡组高于存活组(P<0.05)。结论:血浆BNP、Fib、D-D水平与COPD合并PE的发生发展具有十分密切关系,联合检测上述指标有助于疾病严重程度的判断及患者预后的判断。

Objective:To investigate the application value of combined detection of plasma BNP,FIB and D-D in the diagnosis and prognosis of COPD complicated with PE.Methods:406 patients with COPD were divided into COPD group(n=324)and COPD with PE group(n=82);According to the risk of PE,COPD patients with PE were divided into three groups:high risk group(9 cases),medium risk group(40 cases)and low risk group(33 cases);According to the prognosis,COPD patients with PE were divided into two groups:survival group(76 cases)and death group(6 cases).The levels of BNP,FIB and D-D in peripheral venous blood were recorded.Result:The levels of BNP,FIB and D-D in COPD with PE group were higher than those in COPD group(P<0.05);High risk group was higher than medium risk group and low risk group(P<0.05);The middle risk group was higher than the low risk group(P<0.05);The death group was higher than the survival group(P<0.05).Conclusion:Plasma BNP,FIB and D-D levels are closely related to the occurrence and development of COPD with PE.Combined detection of these indicators is helpful to judge the severity of the disease and the prognosis of patients.
