盆底肌功能训练配合电刺激治疗对产后尿失禁的预防及盆底功能的影响分析

Effect of Pelvic Floor Muscle Function Training Combined with Electrical Stimulation on Prevention of Postpartum Urinary Incontinence and Pelvic Floor Function

目的:探讨盆底肌功能训练配合电刺激治疗对产后尿失禁的预防及盆底功能的影响。方法:将2020年2月—2021年4月来我院分娩的初产妇80例,随机分为对照组和观察组,各40例,对照组采用盆底肌功能训练,观察组在对照组的基础上配合电刺激治疗,产后随访6个月比较两组的尿失禁情况以及干预前后尿失禁问卷简表(ICI-Q-SF)评分,盆底肌力,盆底静息压和收缩压。结果:观察组尿失禁发生率为7.50%,低于对照组的25.00%(P<0.05)。干预后,两组的ICI-Q-SF评分均低于干预前,且观察组低于对照组(P<0.05);两组的盆底Ⅰ类肌纤维和Ⅱ肌纤维收缩时间长于干预前,且观察组长于对照组(P<0.05);两组的盆底静息压和收缩压均高于干预前,且观察组高于对照组(P<0.05)。结论:盆底肌功能训练配合电刺激治疗可以有效预防产后尿失禁,改善产妇产后盆底功能,具有一定的临床推广价值。

Objective:To investigate the effect of pelvic floor muscle function training combined with electrical stimulation on the prevention of postpartum urinary incontinence and pelvic floor function.Methods:A total of 80 primiparas who delivered in our hospital from February 2020 to April 2021 were randomly divided into control group and observation group,40 cases in each group.The control group received pelvic floor muscle function training,and the observation group received electrical stimulation treatment on the basis of the control group.After 6 months postpartum follow-up,the two groups were compared for urinary incontinence,ICI-Q-SF score,pelvic floor muscle strength,pelvic floor resting pressure and systolic pressure before and after intervention.Results:The incidence of urinary incontinence in the observation group was 7.50%,lower than 25.00% in the control group(P<0.05).After intervention,ICI-Q-SF score in both groups was lower than before intervention,and the observation group was lower than the control group(P<0.05).The contraction time of type Ⅰ and Ⅱ pelvic floor muscle fibers in both groups was longer than before intervention,and the observation length was longer than that in the control group(P<0.05).The pelvic floor resting pressure and systolic pressure in both groups were higher than before intervention,and the observation group was higher than the control group(P<0.05).Conclusion:Pelvic floor muscle function training combined with electrical stimulation therapy can effectively prevent postpartum urinary incontinence and improve postpartum pelvic floor function,which has certain clinical promotion value.
