基于“升举三阴法”论治慢性萎缩性胃炎探赜

Discussion on the treatment of chronic atrophic gastritis based on the theory of ‘three yin lifting method’

“升举三阴法”是云南吴氏扶阳学术流派第二代学术继承人吴荣祖教授创新的治法,在临床上可辨治诸多三阴寒化证的病种,吴荣祖教授在传承佩衡先生之吴氏扶阳学术思想的基础上,结合50余年临证经验,总结凝练出具有“升举三阴”之效的“吴萸四逆汤合苓桂术甘汤”。文章首先阐释“升举三阴法”之内涵,即足厥阴肝、足太阴脾、足少阴肾是人体气机圆运动运行的3个关键环节,只要三阴的阳气得到升发,则整个人体气机的圆运动就可以运行不息,阴平阳密,精神乃治;反之则百病丛生。其次基于云南吴氏扶阳学术思想,论述慢性萎缩性胃炎的病因病机,并结合姜莉云的临床经验组成“姜附复萎方”辨治慢性萎缩性胃炎,取得良好疗效,可逆转镜下萎缩及肠上皮化生等癌前状态,并举验案为证,以供同道借鉴和探讨。

‘Three yin lifting method’ is an innovative method of professor WU Rong-zu, the second-generation academic successor of Yunnan WUs’ Fuyang academic school. This method can differentiate and treat many diseases of three yin cold syndrome in clinic. Professor WU Rong-zu, on the basis of inheriting Mr. Pei-heng’s WUs’ academic thought of supporting yang,combined with more than 50 years of clinical experience, summed up and condensed the ‘Wuyu Sini Tang and Linggui Zhugan Decoction’ with the effects of ‘ three yin lifting’. The article first explains the connotation of ‘three yin lifting the methods’, which is that the liver, kidney and spleen constitute the framework of the three yin of the human body. In this framework, as long as the yang of the three yin is raised, the round movement of the whole human body can run continuously, yin is flat and yang is dense,and the spirit is treated;otherwise, there are many diseases. Secondly, based on the academic thought of WUs’ Fuyang in Yunnan,this paper discusses the etiology and pathogenesis of chronic atrophic gastritis, and combines JIANG Li-yun’s clinical experience to form ‘Jiangfu Fuwei Formula’ for the differential treatment of chronic atrophic gastritis, and achieved good results. This prescription can reverse the precancerous state such as atrophy and intestinal epithelial metaplasia under the microscope, and cite the case as evidence, which can be used for reference and discussion.
