癌因性疲乏的中医病因探析

Analysis on etiology of cancer-related fatigue in traditional Chinese medicine

综合古今医家的不同认识,基于中医病因理论对癌因性疲乏的形成原因进行探讨和分析,以期进一步规范其临床辨证分型以及合理选择治法方药提供参考。癌因性疲乏在中医多以“虚劳”“郁证”“百合病”“痿证”论治,其致病原因分别为:癌毒损正、七情内伤、医药之因、病后失调以及饮食劳逸失度;以上诸多致病因素综合作用,导致癌症患者机体呈现“虚”“郁”交织的状态,两者相互转化,互相影响,进一步导致“痰”“瘀”等病理产物内生,诸多病理因素错杂纠结,形成恶性循环,最终导致癌因性疲乏的发生。癌因性疲乏中医病因的明确为其中医药防治提供了思路,有利于促进中医药防治癌症相关性疾病优势的发挥。

Through synthesizing the different understandings of ancient and modern physicians to analyze the causes of cancer-related fatigue based on the etiology theory of traditional Chinese medicine(TCM), so as to provide references for further research on standardizing its clinical syndrome differentiation, and reasonably choosing therapies, prescriptions and medicines.Cancer-related fatigue is mainly treated by ‘weakness fatigue’ ‘depression syndrome’ ‘Baihe disease’ and ‘Wei syndrome’ in TCM. The etiologies of cancer-related fatigue are as follows: consumption of zhengqi due to cancerous toxin, internal impairment due to seven emotions, damage of medical treatment and drug toxicity, maladjustment after illness, improper diet and overwork or over-rest. The comprehensive effect of those pathogenic factors above leads to the interwoven state of ‘deficiency’ and ‘depression’in cancer patients. They transform and influence each other, which further lead to the endogenous of ‘phlegm’, ‘blood stasis’ and other pathological products. Many pathological factors are mixed and tangled, forming a vicious circle and eventually leading to the occurrence of cancer-related fatigue. Definition of the etiologies of cancer-related fatigue will provide thinking and guidelines for the prevention and treatment of cancer-related fatigue in TCM, and shall be propitious to bring the advantage of TCM into full play when preventing and treating the cancer-related diseases.
