导管球囊扩张术联合间歇性经口至食管管饲营养法治疗环咽肌失弛缓的疗效观察

Effect of balloon catheter dilation combined with intermittent oral to esophageal tube feeding on apalasia of muscles around the pharynx

目的:研究导管球囊扩张术联合间歇性经口至食管管饲营养法(IOE)治疗环咽肌失弛缓的临床疗效。方法:将符合标准40例脑损伤后环咽肌失弛缓造成吞咽障碍的患者分为观察组(20例)和对照组(20例),两组均给予包括基础药物及常规吞咽治疗在内的治疗,观察组给予导管球囊扩张治疗,并通过IOE方式进食,对照组则只给予导管球囊扩张术,采用鼻饲方式进食,两组疗程2周,两组患者治疗前后均接受吞咽造影(VFSS)检查,进行包括功能性经口摄食量表(FOIS)、洼田饮水试验、EAT10评分在内的评定,并观察两组患者出现的并发症。结果:治疗组各项吞咽功能总体评分均优于对照组(P<0.05),两组在并发症方面无明显差异(P>0.05)。结论:导管球囊扩张术结合IOE对治疗环咽肌失弛缓能提升疗效,可明显改善患者吞咽功能,加快患者吞咽功能恢复。

Objective:To investigate the clinical efficacy of balloon catheter dilation combined with intermittent oral to esophageal tube feeding(IOE) in the treatment of apalasia of muscles around the pharynx.Methods:Will conform to the standards of 40 cases of brain injury after ring pharyngeal muscle relaxation loss caused by swallowing disorder in patients were divided into observation group(20 cases) and control group(20 cases),two groups were given,including basic drugs and conventional treatment swallowing treatment,observation of treatment group was given catheter balloon expansion,and through the way of IOE eating,control group were only given catheter balloon dilatation,adopts the way of nasogastric feeding,The treatment course of 2 groups was 2 weeks,patients in 2 groups received swallowing angiography(VFSS) examination before and after treatment,including functional oral ingestion Scale(FOIS),depression drinking water test,EAT10 score,and the complications in 2 groups were observed.Results:The overall scores of swallowing function in the treatment group were better than those in the control group(P<0.05),but there was no significant difference in complications between the two groups(P>0.05).Conclusion:Catheter balloon dilation combined with IOE in the treatment of apalasia of muscles around the pharynx can improve the efficacy,obviously improve the swallowing function of patients,speed up the recovery of swallowing function.
