龙胆泻肝汤治疗肝经湿热下注型细菌性阴道病临床研究

Clinical Study on Longdan Xiegan Tang for Bacterial Vaginosis of Lower Energizer Dampness-Heat in Liver Meridian Type

目的:观察龙胆泻肝汤治疗肝经湿热下注型细菌性阴道病的临床疗效。方法:选取60例肝经湿热下注型细菌性阴道病患者,应用随机数字表法分为2组各30例。对照组口服甲硝唑片治疗,观察组给予龙胆泻肝汤口服及中药熏洗治疗。2组均治疗7 d。比较2组的临床疗效、症状消失时间,观察治疗前后阴道pH值、乳酸杆菌数量评分、肝经湿热下注证症状评分的变化与不良反应发生情况。结果:治疗7 d后,观察组临床疗效总有效率高于对照组(P<0.05)。观察组的外阴瘙痒、阴道疼痛、黏膜充血消失时间均较对照组缩短(P<0.01)。2组阴道pH值均较治疗前下降,乳酸杆菌数量评分均较治疗前增加,差异均有统计学意义(P<0.01)。观察组阴道pH值低于对照组,乳酸杆菌数量评分高于对照组,差异均有统计学意义(P<0.01)。2组带下量多、带下色黄、外阴瘙痒、口苦口腻、纳呆、大便干结、小便黄评分均较治疗前减少(P<0.01);观察组7项症状评分均低于对照组(P<0.01)。观察组不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论:龙胆泻肝汤治疗肝经湿热下注型细菌性阴道病可有效缓解症状和改善阴道内环境,提高疗效,且安全性较好。

Objective:To observe the clinical effect of Longdan Xiegan tang for bacterial vaginosis of lower energizer dampness-heat in liver meridian type.Methods:A total of 60 cases of patients with bacterial vaginosis of lower energizer dampness-heat in liver meridian type were selected and divided into two groups according to random number table method,with 30 cases in each group.The control group was treated with metronidazole tablets,and the observation group was treated with the oral administration of Longdan Xiegan tang and Chinese medicine fumigation and washing.Both groups were treated for seven days.Clinical effects and the disappearance time of symptoms were compared between the two groups.Changes in vaginal pH values,scores of lactobacillus amount and symptom scores of lower energizer dampness-heat in liver meridian syndrome,as well as incidence of adverse reactions were observed before and after treatment.Results:After seven-day treatment,total clinical effective rate in the observation group was higher than that in the control group(P<0.05).The disappearance time of vulvar pruritus,vaginal pain,and mucous hyperemia in the observation group was shorter than that in the control group(P<0.01).Vaginal pH values in the two groups were decreased when compared with those before treatment,and scores of lactobacillus amount were increased,differences being significant(P<0.01).Vaginal pH value in the observation group was lower than that in the control group,and the score of lactobacillus amount was higher,differences being significant(P<0.01).Scores of excessive vaginal discharge,yellow vaginal discharge,vulvar pruritus,bitter taste and sticky slimy sensation in the mouth,torpid intake,dry stool,and yellow urine in the two groups were decreased when compared with those before treatment,and the seven symptom scores in the observation group were lower than those in the control group(P<0.01).The incidence of adverse reactions in the observation group compared with the control group,there was no significance in the difference(P>0.05).Conclusion:Longdan Xiegan tang can effectly relieve symptoms,improve vaginal environment and curative effect with good safety in the treatment of bacterial vaginosis of lower energizer dampness-heat in liver meridian type.
