早期床上分级运动对ICU老年机械通气患者肌力恢复的影响

Effect of Early Graded Exercise in Bed on Muscle Strength Recovery among Elderly Patients with Mechanical Ventilation in ICU

目的探讨早期床上分级运动对ICU老年机械通气患者肌力恢复及自理能力的影响。方法选择2020年2月至2021年2月收治于海安市人民医院ICU行机械通气的124例老年患者作为研究对象,将其随机分为观察组(n=62)和对照组(n=62)。对照组行常规锻炼,观察组在此基础上行早期床上分级运动。对比两组患者转出ICU时肌力恢复、自理能力、机械通气时间、住院时间及并发症发生率。结果观察组转出ICU时肌力分级、自理能力Barthel指数评分均高于对照组(P<0.05);且机械通气时间、ICU住院时间及总住院时间均较对照组更短(P<0.05);观察组ICU住院期间深静脉血栓、呼吸机相关性肺炎及总体并发症发生率均低于对照组(P<0.05)。结论早期床上分级运动可有效促进ICU老年机械通气患者肌力恢复,有助于提升其自理能力,减少并发症,缩短机械通气时间与住院时间。

ObjectiveTo investigate the effect of early graded exercise in bed on muscle strength recovery and self-care ability among elderly patients with mechanical ventilation in ICU.MethodsA total of 124 elderly patients with mechanical ventilation admitted to the ICU of Hai’an People’s Hospital from February 2020 to February 2021 were selected.They were randomly divided into the observation group(n=62)and the control group(n=62).The control group took routine exercise,while the observation group took early graded exercise in bed on this basis.Muscle strength recovery,self-care ability,duration of mechanical ventilation,length of hospital stay and incidence of complications were compared between the two groups when patients were transferred out of ICU.ResultsThe grade of muscle strength and the Barthel Index score of self-care ability in the observation group were higher than those in the control group when patients were transferred out of ICU(P<0.05);the duration of mechanical ventilation,length of ICU stay and total hospital stay in the observation group were shorter than those in the control group(P<0.05);the incidence of deep venous thrombosis/ventilatorassociated pneumonia and total complication rate during ICU stay in the observation group were lower than those in the control group(P<0.05).ConclusionEarly graded exercise in bed can accelerate the recovery of muscle strength in elderly patients with mechanical ventilation in ICU,improve their self-care ability,reduce complications,and shorten the duration of mechanical ventilation and the length of hospital stay.
