从水通道蛋白角度探讨活血利水法治疗急性肾损伤的内在机制 被引量:11

Discussion on Mechanism of Blood Invigorating and Urination Promoting Method in the Treatment of Acute Kidney Injury from the Perspective of Aquaporins

摘要 急性肾损伤(AKI)是一种以肾功能快速恶化及代谢废物蓄积为特征的严重临床综合征,中医学认为“水、湿、瘀、毒”是其发生、发展的关键病机,应用活血利水法清除互结之水湿、瘀血、浊毒是中医治疗AKI的重要原则。水通道蛋白(AQPs)是肾脏水液代谢的重要分子机制之一,同时也深度参与了AKI发生、发展过程,并且与AKI的预后密切相关。AQPs是大量活血利水方药的关键作用靶点之一。从AQPs与AKI核心病机“水、湿、瘀、毒”的内在关系出发,探讨活血利水治疗AKI的现代科学内涵,为临床防治AKI提供思路。 Acute kidney injury(AKI) is a severe clinical syndrome characterized by rapid deterioration of renal function and accumulation of metabolic waste.In traditional Chinese medicine(TCM),“water,dampness,stasis,toxin" are considered to be the crucial pathogenesis of AKI.Therefore,blood invigorating and urination promoting method to remove the binding of water-dampness,static blood and turbid toxin holds the key to the treatment of AKI.Aquaporins(AQPs) are important proteins for water metabolism in the kidney,and are deeply involved in the development,progression and prognosis of AKI.Moreover,AQPs also serve as the main target of many blood-invigorating and urination-promoting formulas and medicinals.In this paper,we tried to explore the scientific connotation of blood invigorating and urination promoting method in the treatment of AKI from the perspective of the intrinsic relationship between AQPs and the core pathogenesis of AKI("water,dampness,stasis,toxin"),hoping to provide new ideas and clues for the treatment of AKI in clinical practice.
