基于络脉理论探讨视网膜静脉阻塞微循环的变化

Study on changes of microcirculation in retinal vein occlusion based on collateral theory

基于中医的“络脉理论”和视网膜的微循环概念,探讨在视网膜静脉阻塞(RVO的发生、发展过程中,视网膜微循环的改变与眼底络脉改变的关联性。探究发现在RVO的发生发展中,视网膜微循环的障碍与中医理论中的“久病入络”不谋而合,证实了“久病多瘀”“久病多虚”的络脉理论。建议在临床防治中,谨守证候,灵活选用活血化瘀、利水渗湿、理气化痰,扶助正气等通络之法,加减配伍,斟酌剂量,恢复络脉的通畅,改善视网膜微循环等解剖学指标,从而改善RVO的临床症状,为中医防治RVO提供参考。

Based on collateral theory of Traditional Chinese Medicine(TCM)and the concept of retinal microcirculation,this paper explores the correlation between the changes of retinal microcirculation and fundus collateral in the occurrence and development of retinal vein occlusion(RVO).It is found that the disorder of retinal microcirculation coincides with"Chronic Diseases Transforming to Collaterals"in TCM theory which confirms the ideas of"Chronic Diseases with More Blood Stasis"and"Chronic Diseases with More Deficiencies"in collateral theory.It is recommended that in the clinical prevention and treatment,syndromes are complied with carefully,and methods of dredging collaterals flexibly selected,including activating blood circulation and removing blood stasis,clearing damp and promoting diuresis,regulating Qi and resolving phlegm,and reinforcing healthy Qi.The aim is to resume the free movement of collaterals through addition,subtraction,or compatibility of medicines,and deliberating the doses,thus improving retinal microcirculation and other anatomical indexes,so as to improve the clinical symptoms of RVO,and provide reference for the prevention and treatment of RVO with TCM.
