喉罩在手术室内困难气道中进行处理的临床应用效果研究

Clinical Application of Laryngeal Mask Airway in Difficult Airway in Operating Room

目的:分析喉罩在手术室内困难气道中进行处理的临床应用效果。方法:研究对象来源为2019年3月~2021年3月60例困难气道患者,以随机单双号法分组,设置为对比组(n=30,气管插管全麻)、研究组(n=30,喉罩置管全麻),比较临床应用效果。结果:研究组首次成功率96.67%较对比组80.00%更高,操作时间(41.45±3.48)s较对比组(103.27±10.51)s更少(P<0.05);两组在操作时、苏醒时两个阶段心率、收缩压以及舒张压均存在明显差异(P<0.05),且研究组各项体征波动明显较对比组更小(P<0.05);置入前,两组气道密封压、Pet-二氧化碳分压水平比较均无差异(P>0.05),成功置入、术毕拔除时,研究组气道密封压水平均较对比组更低(P<0.05),Pet-二氧化碳分压水平两组比较无差异(P>0.05)。研究组并发症发生率6.66%较对比组26.66%更低(P<0.05)。结论:在手术室内困难气道之中采取喉罩置管处理方式,能够明显提高首次置管成功率,缩短操作时间,同时,还可以更好维持患者生命体征处于稳定状态,最大程度降低相关并发症发生率。

Objective:To analyze the clinical effect of laryngeal mask airway(LMA)in the treatment of difficult airway in operating room.Methods:60 patients with difficult airway from March 2019 to March 2021 were randomly divided into control group(n=30,tracheal intubation general anesthesia)and study group(n=30,laryngeal mask intubation general anesthesia),compare the clinical application effect.Results:The first success rate of the study group was 96.67%,which was higher than that of the control group,which was 80.00%,and the operation time was(41.45±3.48)s compared with the control group(103.27±10.51)s(P<0.05).There were significant differences in heart rate,systolic blood pressure and diastolic blood pressure between the two stages(P<0.05),and the fluctuation of various signs in the study group was significantly smaller than that in the control group(P<0.05).There was no difference in the level of Pet-carbon dioxide partial pressure(P>0.05).When successful implantation and removal after surgery,the airway sealing pressure in the study group was lower than that in the control group(P<0.05).The level of Pet-carbon dioxide partial pressure was lower.There was no difference between the two groups(P>0.05).The incidence of complications in the study group was 6.66%,which was lower than that in the control group(26.66%)(P<0.05).Conclusion:LMA intubation can significantly improve the success rate of the first intubation,shorten the operation time,maintain the vital signs of patients in a stable state,and minimize the incidence of related complications.
