螺旋CT与磁共振成像诊断腰椎间盘突出症的对比分析

Comparative Analysis of Spiral CT and Magnetic Resonance Imaging in the Diagnosis of Lumbar Disc Herniation

目的:对比螺旋CT与磁共振成像(MRI)对腰椎间盘突出症的诊断价值。方法:回顾性分析本院2019年1月~2020年10月收治的经手术确诊的腰椎间盘突出症患者86例的临床资料,患者术前均行CT及MRI检查,比较CT与MRI检查对86例患者腰椎间盘突出相关征象的诊断情况、诊断准确率、漏诊率及误诊率。结果:86例患者腰椎间盘突出相关征象的诊断结果:CT检查对钙化(89.13%)、积气的诊断率(94.00%)显著高于MRI检查诊断结果(28.26%、28.00%),对椎间盘变性(32.35%)、硬膜囊受压(73.81%)、神经根受压(70.97%)及脊髓变形(44.73%)的诊断率显著低于MRI检查诊断结果(94.12%、95.245%、96.77%、100.00%),差异有统计学意义(P<0.01);MRI检查的诊断准确率(89.53%)高于CT检查诊断准确率(74.42%),误诊率(8.14%)低于CT检查误诊率(22.09%),差异有统计学意义(P<0.05);两种检查方法的漏诊率比较差异无统计学意义(P>0.05)。结论:CT与MRI检查均对腰椎间盘突出症具有较高的诊断价值,相较于CT检查,MRI检查准确率更高、误诊率更低,临床可结合两种检查方法的优点,实现优势互补,从而提高临床诊断率。

Objective:To compare the diagnostic value of spiral CT and magnetic resonance imaging(MRI)for lumbar disc herniation.Methods:A retrospective analysis of the clinical data of 86 patients with surgically diagnosed lumbar disc herniation admitted to our hospital from January 2019 to October 2020 was used as the research object.All patients underwent CT and MRI examinations before surgery,and compared CT and MRI examinations.The diagnosis,diagnosis accuracy,missed diagnosis rate and misdiagnosis rate of 86 patients with lumbar disc herniation related signs.Results:The diagnostic results of relevant signs of lumbar disc herniation in 86 patients:the diagnostic rates of calcification(89.13%)and pneumatosis(94.00%)by CT were significantly higher than those by MRI(28.26%and 28.00%),intervertebral disc degeneration(32.35%)and dural sac compression(73.81%),nerve root compression(70.97%)and spinal cord deformity(44.73%)were significantly lower than the diagnostic results of MRI(94.12%,95.245%,96.77%,100.00%),and the difference was statistically significant(P<0.01);the diagnostic accuracy rate of MRI examination(89.53%)was higher than that of CT examination(74.42%),and the misdiagnosis rate(8.14%)was lower than that of CT The misdiagnosis rate(22.09%)was statistically significant(P<0.05);there was no statistically significant difference in the missed diagnosis rate between the two inspection methods(P>0.05).Conclusion:Both CT and MRI have high diagnostic value for lumbar disc herniation.Compared with CT,MRI has higher accuracy and lower misdiagnosis rate.Clinically,we can combine the advantages of the two examination methods to complement each other,so as to improve the clinical diagnosis rate.
