摘要 目的:探讨根本原因分析(RCA)法在呼吸机维修效率提升中的应用。方法:随机选取2017年1月~2020年12月本院使用的26台呼吸机,于2019年1月实施RCA法呼吸机维修模式,分析2017年1月~2018年12月及2019年1月~2020年12月呼吸机故障模式、发生原因,比较实施前后呼吸机使用效率、维修情况与呼吸机使用过程中不良事件发生率。结果:RCA法实施后开机率、功能期望值评分高于实施前(P<0.05)。RCA法实施前呼吸机维修台数有23台,总维修次数为25次;实施后维修台数为12台,总次数为13次。RCA法实施后维修次数与维修管理费用低于实施前(P<0.05)。RCA法实施前呼吸机使用过程中发生30例不良事件,实施后发生12例不良事件,RCA法实施前后呼吸机不良事件发生率具有统计学意义(P<0.05)。结论:RCA法应用于呼吸机维修中可使工作人员规范使用呼吸机,降低故障发生率,减少维修费用与维修次数,显著提升呼吸机管理质量。 Objective:To investigate the application of root cause analysis(RCA)in improving the efficiency of ventilator maintenance.Methods:26 ventilators used in our hospital from January 2017 to December 2020 were randomly selected.The RCA ventilator maintenance mode was implemented in January 2019.The ventilator failure modes and causes from January 2017 to December 2018 and January 2019 to December 2020 were analyzed.The ventilator use efficiency,maintenance and the incidence of adverse events in the process of ventilator use were compared before and after implementation.Results:After the implementation of RCA,the ventilator opening rate and the functional expectation score were increased(P<0.05).Before the implementation of RCA,23 ventilators were maintained with a total number of 25 repairs;after the implementation,12 ventilators were maintained with a total number of 13 repairs.The frequency of repairs and maintenance costs were decreased after the implementation of RCA.There were 30 adverse events during ventilator use before the implementation of RCA and 12 adverse events after its implementation,with statistical difference(P<0.05).Conclusion:The application of RCA method in ventilator maintenance allows staff to standardise the use of ventilators,thus reducing the incidence of adverse events,reducing the cost and frequency of maintenance and significantly improving the quality of ventilator management.
