富血小板凝胶在经腹腹腔镜腹股沟疝修补术中的应用效果

Application effect of platelet-rich plasma in transabdominal preperitoneal laparoscopic inguinal herniorrhaphy

目的探讨富血小板凝胶(PRP)在经腹腹腔镜腹股沟疝修补术(TAPP)中的临床疗效及应用前景。方法回顾性分析2018年1月至2020年6月于该院行TAPP的60例患者资料,采用随机数字表法分为治疗组(使用PRP)、对照组(使用医用胶),各30例。观察2组术后血清肿、慢性疼痛、补片感染的发生率和费用支出差异。结果2组患者均手术顺利,无中转开腹,术后均顺利出院。治疗组术后血清肿发生率(6.67%)与对照组(26.67%)比较,差异有统计学意义(P<0.05)。治疗组术后慢性疼痛发生率(0)与对照组(6.67%)比较,差异无统计学意义(P>0.05)。术后2组均未发生补片感染。治疗组术后平均费用支出[(12940±594)元]与对照组[(13350±606)元]比较,差异有统计学意义(P<0.05)。结论PRP具备安全可靠性,可提高腹腔镜腹股沟疝修补术的疗效,从而减少术后并发症的发生,并相应降低耗材费用。

Objective To explore the clinical efficacy and application prospect of platelet-rich plasma(PRP)in transabdominal preperitoneal laparoscopic inguinal herniorrhaphy(TAPP).Methods The data of 60 TAPP cases were retrospectively analyzed from January 2018 to June 2020 in Shunde Hospital Guangzhou University of Chinese Medicine,which divided into the treatment group(30 cases using PRP)and the control group(30 cases using medical glue)by using radom number table.The incidence of postoperative seroma,chronic pain,patch infection,and the expenditure were observed in the two groups.Results All the patients in the two groups were operated smoothly,no conversion to open surgery,and all patients were discharged smoothly after the operation.The incidence of postoperative seroma in the treatment group was 6.67%,and that of the control group was 26.67%,the difference was statistically significant(P<0.05).The incidence of postoperative chronic pain in the treatment group was 0,and that of the control group was 6.67%,the difference was not statistically significant(P>0.05).There were all no patch infections after operation in two groups.The average postoperative expenditure of the treatment group was(12940±594)RMB,while that of the control group was(13350±606)RMB,and the difference was statistically significant(P<0.05).Conclusion PRP is safe and reliable,which could improve the curative effect of TAPP,so as to reduce the occurrence of postoperative complications and the expenditure of consumables.
