腹腔镜结肠癌根治术治疗老年局部进展期结肠癌的效果及对患者免疫功能的影响

Effect of laparoscopic radical resection of colon cancer in the treatment of locally advanced colon cancer in the elderly and its influence on the immune function of patients

目的探究腹腔镜结肠癌根治术治疗老年局部进展期结肠癌的效果及对患者免疫功能的影响。方法回顾性选取本院收治的78例老年局部进展期结肠癌患者作为研究对象,依据手术方法将其分为腹腔镜结肠癌根治术组(观察组)与开腹结肠癌根治术组(对照组),各39例。比较两组患者的临床疗效。结果观察组的治疗总有效率高于对照组,差异具有统计学意义(P<0.05)。观察组的手术时间长于对照组,术中出血量少于对照组,术后胃肠道功能恢复时间短于对照组,差异具有统计学意义(P<0.05);两组的淋巴结清扫数目比较,差异无统计学意义(P>0.05)。手术后,两组的CD4^(+)、CD8^(+)均低于手术前,但观察组高于对照组,差异具有统计学意义(P<0.05);两组的CD4^(+)/CD8^(+)比较,差异无统计学意义(P>0.05)。观察组的术后并发症总发生率、复发率低于对照组,差异具有统计学意义(P<0.05);两组的生存率、死亡率比较,差异无统计学意义(P>0.05)。结论与开腹结肠癌根治术相比,腹腔镜结肠癌根治术治疗老年局部进展期结肠癌的效果更好,对患者免疫功能影响更小。

Objective To explore the effect of laparoscopic radical resection of colon cancer in the treatment of locally advanced colon cancer in the elderly and its influence on the immune function of patients.Methods A total of 78 elderly patients with locally advanced colon cancer treated in our hospital were retrospectively selected as the research objects.According to the surgical methods,the patients were divided into laparoscopic radical resection of colon cancer group(observation group)and open radical resection of colon cancer group(control group),with 39 cases in each group.The clinical effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The operation time of the observation group was longer than that of the control group,the amount of intraoperative bleeding was less than that of the control group,the recovery time of gastrointestinal function was shorter than that of the control group,and the differences were statistically significant(P<0.05);there was no significant difference in the number of lymph node dissections between the two groups(P>0.05).After operation,CD4^(+)and CD8^(+)in the two groups were lower than those before operation,but those in the observation group were higher than the control group,the differences were statistically significant(P<0.05);there was no significant difference in CD4^(+)/CD8^(+)between the two groups(P>0.05).The total incidence of postoperative complications and recurrence rate in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);there were no significant differences in survival rate and mortality between the two groups(P>0.05).Conclusion Compared with open radical resection of colon cancer,laparoscopic radical resection of colon cancer is more effective in the treatment of locally advanced colon cancer in the elderly,and has less impact on the immune function of patients.
