充气式保温毯下肢大血管复温在胃癌手术患者中的应用效果

Application effect of rewarming lower extremity large vessels with inflatable thermal insulation blanket in patients undergoing gastric cancer surgery

目的探究充气式保温毯下肢大血管复温在胃癌手术患者中的应用效果。方法选取本院2018年1月至2020年12月收治的110例胃癌手术患者作为研究对象,遵循随机原则将其分为参照组(n=55)和研究组(n=55)。参照组开展常规术中保暖干预,研究组采取充气式保温毯下肢大血管复温干预。比较两组患者的干预效果。结果手术结束时,研究组患者的心率、平均动脉压高于参照组(P<0.05);麻醉诱导后60、90 min以及手术结束时,研究组患者的鼻咽温度高于参照组(P<0.05)。手术后,两组的PLT均减少,FIB水平均降低,但研究组的PLT多于参照组,FIB水平高于参照组(P<0.05);手术后,两组的APTT、PT、TT均延长,但研究组短于参照组(P<0.05)。研究组的术中出血量少于参照组,术后苏醒时间短于参照组,低体温及寒颤发生率均低于参照组(P<0.05)。结论充气式保温毯下肢大血管复温应用于胃癌患者手术中可稳定其术中体征,促进术后苏醒,减少低体温、寒颤的发生。

Objective To explore the application effect of rewarming lower extremity large vessels with inflatable thermal insulation blanket in patients undergoing gastric cancer surgery.Methods A total of 110 patients undergoing gastric cancer surgery admitted from January 2018 to December 2020 were selected as the study objects.The patients were divided into reference group(n=55)and study group(n=55)according to random principle.The reference group carried out routine intraoperative thermal insulation intervention,and the study group carried out rewarming lower extremity large vessels with inflatable thermal insulation blanket intervention.The intervention effects were compared between the two groups.Results At the end of operation,the heart rate and mean arterial pressure in the study group were higher than those in the reference group(P<0.05);at 60 and 90 min after anesthesia induction and at the end of operation,the nasopharyngeal temperature in the study group was higher than that in the reference group(P<0.05).After operation,PLT and FIB level decreased in both groups,while PLT in the study group was more than that in the reference group,and FIB level was higher than that in the reference group(P<0.05);after operation,APTT,PT and TT of the two groups prolonged,but those of the study group were shorter than the reference group(P<0.05).The amount of intraoperative bleeding in the study group was less than that in the reference group,the postoperative recovery time was shorter than that in the reference group,and the incidences of hypothermia and shivering were lower than those in the reference group(P<0.05).Conclusion The rewarming lower extremity large vessels with inflatable thermal insulation blanket applied in patients undergoing gastric cancer surgery can stabilize the intraoperative signs,promote postoperative recovery,and reduce the occurrence of hypothermia and shivering.
