探讨M-CSF、CA153、HPV联合检测在宫颈癌早期筛查中的临床意义

摘要 目的探讨巨噬细胞集落刺激因子(M-CSF)、糖类抗原153(CA153)、人乳头瘤病毒(HPV)联合检测在宫颈癌早期筛查中的临床意义。方法选择2017年5月至2019年8月于本院诊治的192例疑似宫颈癌患者为研究对象,检测患者的M-CSF、CA153、HPV等指标,并以病理检查结果为最终诊断,计算M-CSF、CA153、HPV单独检测及联合检测的灵敏度、特异度、诊断符合率、假阳性率、假阴性率等,并比较不同类型患者的M-CSF、CA153表达水平和HPV病毒载量,采用Logistic多元回归分析以上指标与宫颈癌的相关性。结果M-CSF、CA153、HPV单独检测的诊断符合率依次为68.23%、66.15%、65.10%,与联合检测的92.19%相比较低(P<0.05);与联合检测相比,各单项检测的灵敏度、特异度、诊断符合率较低,假阴性率、假阴性率较高(P<0.05);阳性患者CINⅠ、CINⅡ、CINⅢ及宫颈癌患者M-CSF、CA153水平及HPV病毒载量均高于阴性患者,且随着病情加重呈递增趋势(P<0.05)。经Logistic多元回归分析发现,宫颈癌与血清M-CSF、CA153水平及HPV病毒载量均为正相关,OR值依次为1.022、1.704、1.289。结论M-CSF、CA153、HPV均为宫颈癌诊断的重要指标,其表达水平可随患者病情发展而变化,均属于宫颈癌独立危险因素,联合检测M-CSF、CA153、HPV在宫颈癌早期筛查中有重要价值。 Objective To investigate the clinical significance of combined detection of macrophage colony stimulating factor(M-CSF),carbohydrate antigen 153(CA153)and human papillomavirus(HPV)in the early screening of cervical cancer.Methods A total of 192 patients with suspected cervical cancer who were diagnosed and treated in our hospital from May 2017 to August 2019 were selected as the research objects,and the M-CSF,CA153,HPV and other indicators of the patients were detected,and the pathological examination results were used as the final diagnosis to calculate M-The sensitivity,specificity,diagnostic coincidence rate,false positive rate and false negative rate of CSF,CA153 and HPV detection alone and combined detection,and the expression levels of M-CSF,CA153 and HPV viral load in different types of patients were compared.Logistic multiple regression analysis of the correlation between the above indicators and cervical cancer.Results The diagnostic coincidence rates of the combined detection of M-CSF,CA153 and HPV were 68.23%,66.15% and 65.10%,which were lower than 92.19% of the combined detection(P<0.05).The sensitivity,specificity,and diagnostic coincidence rate were low,and the false negative rate and false negative rate were high(P<0.05).patients,and increased with the aggravation of the disease(P<0.05).Logistic multiple regression analysis showed that cervical cancer was positively correlated with serum M-CSF,CA153 levels and HPV viral load,and the OR values were 1.022,1.704,and 1.289.Conclusion M-CSF,CA153 and HPV are important indicators for the diagnosis of cervical cancer,and their expression levels can change with the development of the patient's disease,and they are all independent risk factors for cervical cancer,combined detection of M-CSF,CA153 and HPV has an important value in early screening of cervical cancer.
