下肢康复训练对股骨转子间骨折内固定术患者术后生活质量的影响

Effect of Lower Limb Rehabilitation Training on Quality of Life of Patients after internal Fixation of femoral intertrochanteric Fracture

目的探究下肢康复训练对股骨转子间骨折内固定术患者术后生活质量的影响。方法纳入2019年1月至2021年1月本院92例股骨转子间骨折患者进行研究,均行内固定术治疗,根据随机数字表法分为常规组46例,予常规术后康复训练;试验组46例,予下肢康复训练,比较两组并发症发生率、术前(入院24 h内)、术后(术后3个月、6个月)髋关节功能、生活质量的差异。结果住院期间并发症比较,试验组坠积性肺炎、骨折愈合延迟、切口感染、下肢深静脉血栓总发生率为4.35%,低于常规组17.39%,比较有统计学差异(P<0.05);髋关节功能比较,术前,试验组髋关节功能(Harris)评分与常规组比较无统计学差异(P>0.05),术后3个月、6个月,试验组Harris评分均高于术前,且高于同时期的常规组,比较有统计学差异(P<0.05);生活质量比较,术前,试验组Barthel指数(BI)评分与常规组比较无统计学差异(P>0.05),术后3个月、6个月,试验组BI评分均高于术前,且高于同时期的常规组,比较有统计学差异(P<0.05)。结论下肢康复训练可减少患者住院期间并发症的发生,并改善其术后髋关节功能及生活质量,应用于股骨转子间骨折内固定术后效果显著。

Objective To explore the effect of lower limb rehabilitation training on the quality of life of patients with femoral intertrochanteric fracture after internal fixation.Methods From January 2019 to January 2021,92 patients with femoral intertrochanteric fracture were enrolled and treated with internal fixation.They were divided into routine group(46 cases)and routine postoperative rehabilitation training group(46 cases)and experimental group(46 cases)which received rehabilitation training of lower limbs,the incidence of complications,preoperative(within 24 hours after admission),postoperative(3 months,6 months after operation)hip function and quality of life were compared between the two groups.Results The incidence of complications such as falling pneumonia,delayed union of fracture,wound infection and lower limb thrombosis was 4.35%in the experimental group,which was lower than 17.39% in the routine group(P<0.05).There was no significant difference in Harris scores before operation between the experimental group and the routine group(P>0.05),at 3 and 6 months after operation,Harris scores in the experimental group were higher than those before operation and higher than those in the routine group(P<0.05).Before operation,there was no significant difference between the two groups(P>0.05)in quality of life,at 3 and 6 months after operation,the BI scores of the experimental group were higher than those of the routine group,there were significant differences(P<0.05).Conclusion Lower limb rehabilitation training can reduce the incidence of complications and improve the function of hip joint and the quality of life of the patients,the effect of internal fixation for intertrochanteric fracture is remarkable.
